At least one of the recent core sets was on Shandalar, and parts of Magic Origins were on Vryn and Kaladesh.
Do single cards count? There are quite a few cards in Planechase and Future Sight on worlds that are otherwise not in game.
Also, I don't think there's ever been a set on Moag, but that's apparently Elspeth's home plane and the site of part of Urza's storyline.

There are quite a few planes we've never visited as part of a block expansion. All these are mentioned on plane cards in the two Planechase editions. There still might be a few more mentioned in the novels, but don't have the decent means to check that out right now.
Ya, just read that on the MTGS Wiki. Apparently both Theros and Arkhos palnes exist in the Multiverse now, because they couldn't clear the Arkhos denomination. Tarkir was also Khanar at first. Hm, if they can rebrand planes at will, we'll probably see more renamed planes in the future.
I'm sure there are lots of planes from Planechase we haven't visited.

The planar types are Alara, Arkhos, Azgol, Belenon, Bolas's Meditation Realm, Dominaria, Equilor, Ergamon, Fabacin, Innistrad, Iquatana, Ir, Kaldheim, Kamigawa, Karsus, Kephalai, Kinshala, Kolbahan, Kyneth, Lorwyn, Luvion, Mercadia, Mirrodin, Moag, Mongseng, Muraganda, New Phyrexia, Phyrexia, Pyrulea, Rabiah, Rath, Ravnica, Regatha, Segovia, Serra's Realm, Shadowmoor, Shandalar, Ulgrotha, Valla, Vryn, Wildfire, Xerex, and Zendikar.
I slashed out the planes we visited as a theme of a whole set or block, and autocarded examples of those who were just part of a set (for example, Vryn and Kaladesh on Origins, or Phyrexia and Serra's Realm in Urza's Saga.)
At the very end of this video you can see some very telling signs that Elspeth is not completely done since that mask looks a bit like Godsend. This however is all speculation in all actuality, and I digress from the original forum topic.

She is not out of the picture simply because of how death works on Theros. If they wanted her to be gone for good, they would have to explicitly state that she was somehow completely obliterated, and not just sent to the underworld. On the other hand, I don't think she will stay mono-W after the return and Sorin is the prominent WB walker now.
Also, can she planeswalk away from Theros underworld or does she have to return first?

She is unable to planeswalk out of the underworld. Also, last we saw of Dack Fayden, to my knowledge, he was heading into the underworld on Theros. So he may be coming into the fold and meeting Elspeth and he may have something to do with the story of the next block that takes place on Theros.

You don't see Elspeth there, just her Godsend sword near the tomb and the mask above the tomb that lights up. That's supposed to tell us she's coming back.
Zombies when they return lose their souls and the bodies become The Returned and the spirit becomes Eidolon. Neither knowing where the other exists. The only problem, though, is we don't know what happens when an Eidolon and a Returned fuse together.
So we may get a fully intact Elspeth through the story line as a zombie chick.

Tamiyo should be in Eldritch Moon, because she already appears in 3 pieces of artwork from SoI: Catalog, Avacyn's Judgment and the extended art of Anguished Unmaking. The other PW we could see in ENM is Tibalt.
L.E.: MaRo stated on Blogatog that Tibalt won't be in ENM, not sure if that's true because MaRo is known to feed misinformation occasionally. Instead, we could be getting a new Liliana.
