So, we know from this article that in Magic Origins we will have cards that come from up to 10 different planes, 5 home planes for our 5 planeswalkers and 5 of the first planes each of them first went to. Currently we have 3 revealed since Gideon's story has been leaked a bit.
Gideon: From, Theros. To, Alara(Bant)
Jace: From, ?. To, ?
Liliana: From, ?. To, ?
Chandra: From, ?. To, ?
Nissa: From, Zendikar. To, ?
I also have a theory that 5 of the planes we visit will be the planes from the "Soul" cycle that we had in M15. I believe they will relate to the Planeswalker of that color but I don't think it needs to be their home plane, I think it can be the first plane they go to. So far this is still in the realm of possibility since we know both Gideon and Nissa do indeed confirm Theros and Zendikar. That would mean Jace, Liliana and Chandra all have something to do with Ravnica, Innistrad and Shandalar respectively. Now Jace and Liliana both have obvious ties to Ravnica and Innistrad but I don't believe it's ever directly stated that either of those planes were were they lived on or first went to but I think Ravnica makes sense for Jace's first walk and I could see Innistrad being Liliana's first walk as well. Chandra makes enough sense for Shandalar. So now we're speculating on 6 planes,
Gideon: From, Theros. To, Alara(Bant)
Jace: From, ?. To, Ravnica
Liliana: From, ?. To, Innistrad
Chandra: From, ?. To, Shandalar
Nissa: From, Zendikar. To, ?
We also know that while there will mostly be planes we already know in the set, there will be a few new ones as well. How many? Beats me, but I'd say 6 established planes already equates to "most" so it's possible that all 4 of the "unknown" ones will be new planes but it's more likely we'll get one or two older planes as well. Which planes do you think we'll likely see? Which planes do you want to see? Is there any lore or fluff I'm unaware of the points to or suggests where the characters first went to? All that and more can be discussed here.

Edit-(June 3rd, 2015)
As of today's Uncharted Realms we have our planes.
Gideon: From, Theros. To, Alara(Bant)
Jace: From, Vryn. To, Ravnica
Liliana: From, Dominaria. To, Innistrad
Chandra: From, Kaladesh. To, Regatha
Nissa: From, Zendikar. To, Lorwyn
For anyone new(er) to the game here's what I know about the lesser known planes up here.
-Vryn was once depicted in a Planechase card Trail of the Mage-Rings. I don't think we know much more about it other than it looks mountainous and clearly has some manner of artificers living there.
-Dominaria was the OP (original plane) as I understand it. Most of the early sets all took place on Dominaria and the Serra Angels used to call it "The Wheel" because events that happened there usually end up effecting all the planes. Figures that's where Lili would be from :P.
-Kaladesh is the "newest" plane from what I can tell. At least, I'd never heard of it until that fake Chandra card. Others can correct me if I'm wrong (really about any of this).
-Finally, Regatha... They have cats. They also have a culty organization called "Order of Heliud" that uses flames of white mana to "purify" people. There was a book about Gideon and Chandra there. That's the extent to which I've heard about it.
Should be a fun set!
I don't think that there was any indication that Jace first walked to Ravnica or Liliana to Innistrad. They didn't seem to be particularly familiar with the plane. The planes appearing in this set will probably be the ones focused on for the next few blocks, or most recently focused on, so I couldn't see a return to something like Ravnica or Innistrad anytime soon.
I think it was stated somewhere, years ago, that Nissas first planeswalk was to Mirrodin. Not sure on that though. Jaces is anyones guess since he apparently erased some of his own early memories, hence why he has no idea where he's from.

I believe Nissa's bio stated that she visited Lorwyn (not necessarily her first Planeswalk).
I believe the vast majority of them will be known planes (around 7 or 8). I agree with Innistrad, Ravnica and Shandalar tied to Liliana, Jace and Chandra. I would be surprised if Dominaria isn't one of the 10 planes. It would be cool to see Old Phyrexia, but it is kinda unlikely I guess.
Yeah, wasn't sure if anything in DOTP ruled it out as a possible plane she may have gone to first. Also given the Gideon/Theros thing how much can we trust the earlier DOTP games.
It is confirmed most planes are going to be old, with some new. For me that means a 6/4 or 7/3 division.
It was also explicitly said Chandra's native plane is brand-new. It can't be Shandalar.
The Soul cycle was a reference to Duels of the Planeswalkers 2015 - those were the six (there was a colorless Soul of New Phyrexia) planes visited in the game. I very much doubt they'll base another game around this exact same group of planes just so the Souls could be a reference again.
So I just want to throw out there to everyone saying the "Soul of..." creatures not being a clue missed the first person to posit that theory's post. They indicated that they thought Jace, Lilly, and Chandra would go TO those planes as their first walk, not that they would be FROM those planes.
Mind you, I don't think that is accurate, but considering this is a speculation thread, as fine a logical leap as any to decide what planes we might be seeing in the set.
I'm not entirely sure where they will have some of these 'walkers be from or where they walked the first time, but I would be willing to bet that at least one new plane will be the focal plane of the post-BFZ block. If I had to make a more specific bet, it would be that Wizards tortures us all by making it Jace's home plane. Unless it turns out he is ACTUALLY from Ravnica, and erased his own memory of being from there, then I would say not that.
I dont think Lilianas first planeswalk is going to be to Innistrad. Innistrad seems to much like her homeplane we saw in the webcomics. They more than likely want contrast from home plane to plane after first planeswalk. Much like Ajani and Gideon. Granted Ajani never actually left his homeplane, Naya and Jund where noticeably different and Gideon would easily notice the difference between Theros and Bant. And even with Nissa it would be easy to see the contrast of a plane with no hedrons to know shes not on Zendikar anymore. They'd likely keep with that. A first planeswalk is dramatic so to show that you need to contrast the homeplane from the destination as much as possible.

If you look at the image on this thread I think Jace's first plane will be Ravnica. At least from the small image, it seems to be Ravnican architecture behind him.
Well, maybe it's wishful thinking, but maybe it's Fiora: a mind mage, in a city full of intrigue and plotting? What better way to start your new life? (After all, Jace's known to have extorted nobles for riches, in his youth)
It could also be a known but never visited plane, like Kephalai. Guess we'll see! (But Ravnica does seem like a good and logical choice from a storytelling point)
MTGS Wikia Article about "New World Order"
Every time I read a comment about "Well if this card had card draw/trample/haste/indestructible/hexproof/life gain...", I think "You're missing the point." They're armchair developer comments that fail to take into account the card's role in the greater Limited and Standard environment. No, it may not be as good as whatever card you're comparing it to. There's a reason for that. Not every burn spell is Lightning Bolt, nor does it need to be or should be.
PSA to everyone who keeps forgetting about the Reserved List:
You're on a website dedicated to talking about MtG. You're only a few keystrokes away from finding out what cards are on the Reserved List. You're also only a few keystrokes away from finding out why some cards on the Reserved List got foil printings in FtV, as Judge promos, or whatnot, as well as why that won't happen again. Stop doing this.

"I don't remember home," Beleren said quietly, unbidden.
"What?"
"You talked about growing up in Ravnica. A lot of my memories from my childhood are gone. Chopped up in my head into a few impressions. Most of what I remember begins here, on Ravnica. I'll never have roots here the way you do, and I admit I'm off to other planes a lot. But I think of myself as Ravnican to the core, too."
Straight from the mothership. We already knew Jace chopped up his early memories but judging from this little snippet of this weeks article I'd say we have a bit more evidence pointing towards my earlier theory that Ravnica was indeed the first plane Jace walked to. This doesn't really confirm or deny my larger theory that the Soul cycle was a hint towards the planes we'll see in Origins but it does at least give us a bit more proof that Jace lept to Ravnica and we're likely to see it as one of the Planes in Origins. Not a huge suprise (if true), it's a popular plane. I'd be more suprised if they didn't involve it.
It makes a lot of sense considering the first planeswalk is usually associated with moments of great turmoil and is quite likely that Jace's memory was erased due to those moments.
Anyway, as many users have said, Ravnica, as the most popular plane, will most likely be among the 10.
If you read the latest story with Ral and Jace talking, it is clear that Jace is not from Ravnica, but that he feels that it is his home and he has lived there for some time now. It is quite probable that the implication is that Ravnica was the first place he walked to.

I wouldn't consider the first plane you walked to to be the plane you grew up in. If anything this disproves the theory.
How so? Because it seems not 100% certain, but at least fairly apparent that Jace wasn't born on Ravnica but it's his earliest memory and that points to it being the first plane he visited to me.

Because even if it's your earliest memory, you didn't grow up there. If I become an amnesiac today, that doesn't mean I grew up again. At the very least I could figure out that if no one here knows me, this isn't the place I'm originally from. If he isn't from Ravnica, and he didn't planeswalk as a very young child, he couldn't have grown up on Ravnica.
Except he didn't grow up there, he just landed there, probably met one or two people, got a hold of his powers and left. Jace is a wanderer but he always comes back to Ravnica for some reason. That's even where Garruk went to find him when he wanted to question him on Lilianna's whereabouts.
I'm not claiming certainty but considering we have the origins set coming out in a little over a month this seems like Wizards way of saying Ravnica is important to Jace. Continuity has never been their strongest point but I guess time will tell. I'm putting my wizards hats on Jace walking to Ravnica.

"You talked about growing up in Ravnica." You can't grow up in a place that you weren't raised in. Either he planeswalked there as a young child and was raised there, which is highly unlikely, or he's originally from there, in which case it can't be his first planeswalk location.

That line is directed at Ral. Ral is native to Ravnica. Jace is definitely NOT from Ravnica but I wouldn't feel uncomfortable saying that his first walk was there. I mean it's where he feels at home and we know that RTR is not his first visit there. At the beginning of The Secretest he already knew Emmara.
Also I don't think Innistrad is Lili's first walk. Just speculating here but I feel like we could see one of her other demons. I mean we only know of Kothophed and Griselbrand and she formed pacts with four demons.
I thought Nissa only went to Lorwyn after Sorin had already started teaching her about black magic. I could be very wrong on that though.

Actually, the article says nothing about Jace growing up on Ravnica. It says that Ral grew up on Ravnica, and that Jace is somewhat jealous of that because he doesn't have a single memory of when he was growing up. It says that what he remembers (a.k.a. after he first planeswalked and erased his memory.) starts on Ravnica. It's pretty obvious that Jace first planeswalked to Ravnica from the article.
Anyway, I think that Lili's planes will be home=dominaria (she was pre-mending, so she was able to be on dominaria before the mending.) first walk= a new plane. Nissa's could have to do with Muraganda, the dinosaur plane, and obviously Zendikar (not necessarily in that order.) We already know Gideon's, but I'm not sure on Chandra's planes.

I don't think we will go to Shandalar again (in a core set) soon because I think people would expect Slivers which we got in back to back Core sets.
The soul cycle is NOT a hint, it's coincidence; there's no way Jace is a native of Ravnica or Lili of Innistrad.
And at least one of the planes we'll see, maybe more, are confirmed to be never before seen.
