Did anyone else notice that the token/packaging cards feature an M15 advertisement with an Elspeth profile different than what we have seen before? While Garruk will be the face of M15, I speculate that Elspeth will replace Ajani in M15 as the mono white walker. I think also that Theros will be Ajani's last block, with his death being part of the "real consequences" hinted at by developers.

I think it is unlikely that Ajani dies, but I could see him getting replaced by Elspeth (or even Gideon) as the iconic white planeswalker.
The three of them have been fighting for that role, with no clear winner. Unlike the rest of the Lorwyn 5, Ajani didn't stick as much. Maro's theory is that he isn't "human" enough. There's also the fact he hasn't been relevant to the story since Shards of Alara. Even in Theros, he's playing a supportive role while another white planeswalker takes the spotlight.
I do not have a pic. It didn't come in my pre-release pack but I saw it in a pile from someone elses. Does anyone else have a copy of this card? It is an M15 advertisement with a pic of Elspeth.

While it would be the first block to return to somewhere that actually had planeswalkers the first time around, I don't think it would be planeswalker themed. I don't think they'd wanna abandon the land theme it originally had.
That said, I could still see them killing off Ajani, but don't particularly want them to. I mean, say we get another heavily multicolored block. I'd love to see naya Ajani(as well as jund Sarkhan) before he's killed off.

There was already one change in the M1X planeswalker roster, when Gideon and Sorin replaced Ajani and Liliana for the first time. Neither Ajani nor Liliana died that time. Why should Ajani die now?

Do I see a Return to Alara in the near future making this happen?

They don't judge walkers solely by how their cards fared, their popularity as characters has a say in it as well.
The next Fall set is Sarkhan's home plane (described as a place "torn by warlords", and the set is "Warlords of Khanar", as Conspiracy proving to be a product has shown), so we'll be seeing him again one way or another I think.
MaRo said Huey block is a bottom-up design, and has some kind of unique specific block structure they came up with and wanted to find a home for. I wonder if it might involve plane-hopping? Be kind of cool to have one theme travelling across different planes, like Urza's/Masques blocks but more new-style (one world a set). Bringing Sarkhan and Bolas back into things seems like a good time for something like that.
As for Ajani, maybe he ascends to Godhood in order to stop Xenagos? The Leonin do need a representative...

Firstly, the leonin don't need a god. That's literally their entire culture, they actively choose not to worship the Gods of Theros because they've been able to thrive without doing so.
Secondly, that has nothing to do with Ajani's motivations as a character.
He's a peacekeeper, he doesn't want to be venerated as a hero.
If Elspeth is indeed replacing Ajani in M15 chances are they're just going to reprint Elspeth Tirel and be done with it as that is a card simplistic enough to throw into a Core Set.

The last time they replaced a "Lorwyn 5" planeswalker (Ajani and Liliana for Gideon and Sorin), it was by reprinting old versions of them, so I agree if Elspeth is in M15 it will be as a reprint.
If they were planning in reprinting Elspeth Knight-Errant for Standard she wouldn't be in Modern Master - just like Thoughtseize wasn't. So yes, Elspeth Tirel looks like the most likely version (maybe with a small tokens subtheme for white on that set).
