They probably just had too many walkers from a story perspective. Sorin, Nahiri, Tamiyo, and Lili were all central to the story and couldn't be replaced, and Jace was there as the obligatory member of the Gatewatch. That leaves Arlinn, and since she was the Werewolf walker they'd been trying to make for a while (and the only new walker), it's likely they decided to fit her in instead of Tibalt. That said a new Tibalt could have been a really good tool for madness in Standard.
I get that he wasn't especially popular with pro players, but I remember an article where Maro stated he was surprised at his popularity and "we might be seeing him".
I just feel in a format, at his home plane, with madness floating around and chaos everywhere it would be the perfect opportunity for woc to do him justice.

actually most players DO like Tibalt as a character. They don't like his CARD, because it was downright terrible. a +1 that did more harm than good, and needed to be used twice to even try the second ability. And the second ability wasn't worth using, -4 to do damage to a player equal to the amount of cards in hand?! that ability would only be able to hit a control deck for more than 3 damage by the time you could use it, and losing more loyalty than the amount of damage you do on a damage ability is beyond terrible. his ultimate however was fantastic, if you could ever get to it. wizards wanted to try the 2 mana walker so they made it SOOOOO bad that, not only was he unplayable, but downright hurt YOU to play him. thats why people mock Tibalt all the time, because it is undeniably the worst planeswalker ever printed. people talk ALL THE TIME about their hatred for Jace, but we keep getting him. But Tibalt the character, is one of the most fascinating villainous characters in mtg history, truly evil. He could make for a great villain in a future block, so long as they not only make a playable version of him, but a good version of him.
If price is a factor, Tibalt is actually more expensive than The Living Guild Pact, despite the former having two printings (not counting the SDCC promo version of Jace), so I mean..Tibalt is arguably not the worst walker ever printed.
Okay, lets compare tibalt and living guild pact. both have terrible +'s that don't give you card advantage, and needlessly put a card in the GY. but jace lets you choose which one. so jaces + is better. as for middle abilities both are super overcosted, jace can bounce a card, but tibalt can do damage, and as bad as it is, it can win you the game. so tibalt has the better middle ability. on to the ultimates. both ultimates are awesome. tibalts can win the game, but it is super conditional, and depends entirely on your opponent having a decent board presence. jace however, not only shuffles all GY's and hands into libraries, which hurts any control decks or GY decks, but also draws you a new hand and opponents are left top decking. sorry but jace has to win the ultimate ability challenge.
my math here says jace has win, 2 abilities to 1. they are both really bad walkers, and jace's price may be lower, but I think that may have more to do with it coming from a core set that likely had a bigger run, so there are more living guild pacts on the market

As for why he didn't show up, only so much space, and Tibalt doesn't strike me as being really key to the story. He can always pop up somewhere else, it's not like he has to be anchored to Innistrad. Jace/Liliana were necessary (though more of a meta reason), Sorin/Nahiri are central to the entire conflict, Tamiyo has been studying Innistrad, and Arlinn already made it six.
I'm ok with Tibalt not being in this set. Save him for next time around and have him be really good with a heavy demon/devil theme to innistrad with more ghosts and ghoulies to add to our tribal decks. Since the angels are gone relatively what's to keep the demons at bay.
His character seems to be somewhat cowardly, so it probably shouldn't come as much of a surprise that he would jet at the first sign of things on Innistrad turning sour. Not much fun to be had for a sadistic pain mage on a plane where everything is erupting into gibbering masses of tentacles.

I don't really see how it would be. Would just be kind of disappointing. Even if I'm not happy with Return to Innistrad overall, I don't think wrecking another plane would be good. Hoping that we will see future Returns be less... extreme in the shift.

Well, he's not exactly altrustic. So there isn't much reason for him to be on Innistrad right now. Everyone's either going insane, turning into Eldrazi or undead. So he wouldn't have any interest in being there. And he certainly won't be taking an oath.
