Seriously, this guy is nuts. I've been playtesting with him in puresteel and he is just insane. He essentially serves as puresteel 5-8 and has amazing synergy with the pre-existing deck. I honestly believe his presence is gonna catapult puresteel into the deck to beat post-rotation
Mentor of the Meek is obviously good even from a casual glance. That doesn't qualify as a sleeper.
If I had to choose something Blasphemous Act seems like it could see some play especially if they add any more cards that actually care about coverted mana cost like Mindshrieker or Heretic's Punishment.
My top 3 sleepers are fairly well known... but I don't think that they are getting the respect they deserve.
3. Curse of Death's Hold - This effect is incredibly powerful and generally underrated.
2. Tree of Redemption - Unique. I see this guy just causing problems. Many different uses.
1. Bloodline Keeper - Fails Dismember/Lightning Bolt test, but a token producing machine that more or less just wins a game if he flips. Very powerful and well costed. (I could even see him used like Grave Titan.)

If he counts as a sleeper I'm pretty shocked.
I presume being revealed on the last day meant people might have missed him...but he'll carve out a very nice niche for himself as a sideboard choice for dealing with red. He will definitely see plenty of play.
Civilized Scholar is one of the best uncommons in the set, and is really good in any aggro deck that wants to cast cards with/out of the graveyard. I can see a modern deck with this, Forbidden Alchemy, Bloodghast and a bunch of other graveyard-matters type black creatures maybe with Crypt of Agadeem.
Mindshrieker is hugely underestimated. I could even see this in legacy, a control deck that uses topdeck manipulation to mill Emrakul, the Aeons Torn, recycle all of it's control cards and swing for 16.
Bloodgift Demon? More like Valuegift Demon. It's a flying threat and a card engine in one. And this time around, we won't have Baneslayer Angel keeping any demons away.
I think Kessig Wolf-Run is seriously undervalued, which is good for the wiser. I'm getting a playset for sure. It ensures later in the game that any of your creatures become a threat. I'm also looking at Moorland Haunt, should be more valuable later on. That's an aggressive cost to get a 1/1 flying Spirit, even if you have to exile a creature from your graveyard.
I agree about Heartless Summoning being a sleeper as well. That card is just waiting to do some unfair things.
Whether or not it worked does not validate the play. That's hindsight probability. Let's say I offered you a bet -- I'm going to flip a coin and if it's heads, I'll give you 1 dollar but if it's tails, you'll give me 2 dollars. This is obviously a terrible bet for you. Accepting it is a bad decision. You can't justify it afterwards by arguing that you won the flip, therefore you made the right decision.

Kruin outlaw seems sick... There are already some decks abusing t1, memnite, mountain Infernal Plunge, geosurge, titan. I think t1 kruin outlaw is pretty good too. Even without the acceleration, the card is just beautiful, very strong and the art is awesome.
Kessig cagebreaker, is jaw dropping. Limited nuke basically. A bit slow in constructed and is also quite restricted because it has to have dudes in the graveyard to work out, and a deck using this guy has to have a lot of 1 and 2 drops for it to work out to the max i think.
Tree of redemption is pretty awesome. Very strong, and for 4 mana, let us remember it blocks almost anything, even a blightsteel colossus for one turn. Red decks are bound to have a bad time dealing with this and although it´s not a complete substitute for the baloth (green needed some life gain for 4 mana), it´s pretty epic.
Mentor of the meek. I really like this card... if it were 1/1 or costed 4 it would NOT be the same (duh). But 2/2 for 3 looks soo nice. My first thought was elspeth tirel, and then i figured it might be a bit too slow, and then realized... yeah, it´s probably not that good... either way, relatively cheap card drawing in magic has always been powerful, this might just get there...
Number one contender for the best sleeper card in innistrad HAS to go to the priest. This dude has insane potential. And you don´t really have to look much further to realize that this guy can be abused, in this set alone there is mausoleum guard, doomed traveler, Midnight Haunting etc.
I for one, am digging this with blade splicer, that has quickly become my favourite creature in scars block. T2 this T3 splicer, and virually speaking, they can´t really attack you without risking a 5/5 flyer to spontaneously generate on your side of the table. If the board gets to that, and you untap with removal or any crature in hand (another splicer, phantasmal image, solemn, whatever) + 1 mana open for a dismember, they´ll feel the pain. And even if they kill your dude, you still have the other dudes, and if they played a dude themselves, just dismember it eOt and make a 5/5 while your at it. Sick card.
Mindshrieker - cheap, versatile. Can be used for controlling your opponents draw, dump fatties in your GY, do damage by himself, has evasion so eventually a good sword bearer.
I'm revising my one from Geist-Honored Monk (people seem to be saying he's fairly playable these days) to Curse of Death's Hold. I haven't seen much talk about it, but the effect is excellent.
Not exactly a sleeper, but I suspect Parallel Lives will be more popular than initially expected. Seems like a casual hit, but only 'cute' in Standard, but the effect is no joke.

Some sort of U/B summoning deck could be pretty decent, or U/B/x.
Heartless summoning with Sturmgeist, Psychosis Crawler, Bloodgift demon, Consecrated Sphinx? altars reap and oculus may find a home here.
I got a foil heartless summoning at the prerelease which was completely unplayable, plus I've always been a fan of psychosis crawler.

you are correct. There are only a couple of ways to get a turn 1 titan

Chancellor wont work, doesn't give you red mana, geosurge needs 4 red.

Noticed a few people have mentioned Stromkirk Noble. He's really, really bad in comparison to Goblin Guide...But he's really, really good in comparison to every other red one-drop
Also, the 'can't be blocked by humans' has not once been relevant after playing a ton of games with BR vamps and RDW...It's a nice plus to have and sure it might matter sometimes, just hasn't happened for me yet.
Heartless Summoning, definitely the #1 sleeper. Its value is at around $3 right now, but I've seen some pretty insane decks doing things like...

You can recklessly ramp into solid creatures, and even if you're tapping out and eating removal, you're still getting value, while your opponent is down a card. I wouldn't say HS.dec is the dominant archetype but it's definitely going to be Tier 1. In any case, it greatly outclasses every aggro deck I've seen so far.
Don't know if you guys saw this already, but in Craig Wescoe's 9/23 article "Financial Predictions for Innistrad" (link: http://magic.tcgplayer.com/db/article.asp?ID=10025), at the end of the article he has a list of 24 rares that he says are garbage and doesn't expect any of them to rise above $1. Then in the comments Brian Kibler said "I have already preordered nearly 200 copies of a card on your 'not a good buy' list."
Devil's play is very good, better than most people would give it credit for.
I think the new black edict for 3 is surprisingly good as well. It may be a very important piece against any deck which is dependent on swords to win ( especially with hexproof critters)

Unburial Rites is a card most people haven't given too much attention, but it's an extremely powerful card.
Sorry but are you serious? Everyone is talking about (ab)using that card and the fact that it enables a solar flare/reanimator type of deck is obscene. It's really good in conjunction with forbidden alchemy; turn 3 alchemy, turn 4 sun titan into phantasmal image into something spicy or you just get 12 power worth of dudes that can recur cards that hit the bin later. The card is very likely going to make a splash in standard.
Heartless summoning is also supremely underrated right now. 2 mana solemn simulacrum? How is this fair again, this business about getting solemn on turn 3 and a titan on turn 4, providing so much tempo and card advantage.
My sleeper is champion of the parish that's my pick for going over 5 dollars in the future. Get them while they're cheap folks.

Bloodgift Demon as #1 sleeper, Sturmgeist as #2. I'd use either as a control win condition, although I prefer the Demon. A 5/4 flying beater for 5 that helps replenish your hand seems good to me.

Skirsdag High Priest is definitely a standard card worth looking after. A creature this cheap with the potential to pump out 5/5s in the same set as Village Bell-Ringer and Disciple of Griselbrand seems pretty good to me.

Boneyard Wurm looks pretty good if you doing a self-mill aggro type of deck. He is Splinterfright's cousin.
