Claustrophobia. Because I'm Claustrophobic. Also has some of the creepiest art for the same reason.

If I could "like" or "upvote" you right now, I would.
Quote from Boing »
Into the Maw of Hell is a name so awesome, it deserves its own song.
It would be about a world of brown spikes and red fire, where men fight and scream and become blurry masses. Then it would change to somber, we learn that this glorious place exists only in Raymond Swanland's head. To visit it, you must enter the maw of Hell itself.
1. Grasp of Phantoms.
I keep picturing someone standing next to a door as they hear this faint knocking sound on the wood. As the person puts his ear to the door to listen closely, it stops. moments later, the door bursts open as several ghastly clawed hands reach for him, grab him, and pull him into the darkness screaming. c.r.e.e.p.y.
2. Bump in the Night.
This one, once upon a time, would've reminded me of every creepy thing lurking around in the dark that I feared when I was a child. Now it just makes me think of Slender Man. =P
3. Endless Ranks of the Dead.
Beautiful, yet ironically creepy art. The undead horde pressing up against the stained glass window (possibly depicting Avacyn) brings about this sense of doom that few other cards quite portray.

Claustrophobia has that odd nose that makes me laugh every time I see it.

My Commander decks:
Chandra, Torch of Defiance - Oops! All Chandras.
Prime Speaker Zegana - Draw for Power.
Pir & Toothy - Counterpalooza.
Arcades, the Strategist - Another Brick in the Wall.
Zacama, Primal Calamity - Calamity of Double Mana.
Edgar Markov - Vampires Don't Die.
Child of Alara - Dreamcrusher.
Curse of the Bloody Tome, probably just b/c it reminds me of Pan's Labyrinth where the book starts bleeding when Ophelia's mom almost miscarries.
Brain Weevil. There's a big freaking bug on your face while you're sleeping, and it's going through your ear and NIBBLING ON YOUR BRAINS!!! How can that not be in the Top 5 for creepy?

Love. Forgive. Trust. Be willing to be broken that you may be remade.
Sensory deprivation - The Art ist just gruesome *shudder*
One Eyed-Scarecrow - I never liked them, they give me the creeps... They remind me of a Swiss Myth http://www.imdb.com/title/tt1296077/
Claustrophobia should've been a curse, just to really mess with players' heads... wizards dropped the ball there I reckon. Needed something a little bit constricting like:
claustrophobia
enchantment - curse
After the first card drawn in a turn, enchanted player cannot draw cards for the rest of the turn.
or something along those lines

Cloister Youth/Unholy Fiend - Both art and flavors are dark. It reminds me of Exorcism (probably where the idea came from).
Claustrophobia - One of the worst ways to die. I cannot enjoy animals or anyone being treated thus, let alone myself.
Delver of Secrets/Insectile Aberration - Ack, Human Insect.
Hysterical Blindness - The flavor tells unfortunate truth. Scapegoating is very foolish, and very satisfying.
Sensory Deprivation - It's an old style punishment in the past. Human knows how to torture.
Gruesome Deformity - I like the irony there.
Rotting Fensnake - Photographic realism.
Village Cannibals - Awesome mood. You really see what's coming.
Blasphemous Act - The flavor text shows much peril of the situation.
Desperate Raving - Innistrad has a lot of 'madness = truth' theme in it.
Furor of the Bitten - See above. Another mental crisis taken form.
Creeping Renassance is a little offputting as well for some reason.
Biggest disappointment is dead weight. Expected the guy to be chainned to dead bodies, forced to drag them around.
Done by Rivenor of Miraculous Recovery signatures!
Unbreathing Horde looks like a level of House of the Dead
i love zombies, wish more were playable competitvely since their art is always (well normally) good
I agree. Also, Gallows Warden, Spare from evil, Dream Twist, Mirror-Mad Phantasm, Sturmgeist, Abbatoir Ghoul, Army of the Damned, Maw of the Mire, Burning Vengeance and some others.
Abbatoir Ghoul is the most manacing creature.
Harvest Pyre reminds me of the Wicker Man.
Creepy doll... is creepy.
Butcher's cleaver looks like it has been used on human fesh for some reason (maybe because of the Abbatoir Ghoul art).
I'm a Spike Vorthos - I love lore and flavor, but only if the cards are competition worthy.

This. 1000 times this. Something about the art and the flavor text just creeps me the $#@! out.
