Is ironically a pretty terrible commander as you would need to cast him, target him, and then find one of the few ways to cast him from your library in mono-blue... with a surprising lack of cards that would put him on the top of your gigantic library.
Honestly works better in the 99 for certain U/G or U/R decks... though those decks likely have better options available.
This card seems pushed. A worst it's an Elvish Visionary without a relevant creature type but with a silly shuffle ability. Visionary's solid, so that's an awfully high floor.
Multiple elvish visionaries can coexist for you. Multiples of this cannot.

I'd love this card if it had the Norin ability, or just when a spell is cast. You almost lose card advantage because you cast a spell on Fblthp and then later get to maybe draw 2 if you have ANOTHER card to allow that.
You cast Fblthp and draw a card, then use that card to shuffle back in. Now you're down at least 2 mana and may never see it again. Maybe with Bolas's Citadel, you pay 2 life for 2 cards and then move on to cast more things?
EDIT: I guess just getting the initial 2 and a blocker is value, if you never plan on reusing or abusing.
This is just begging to have a janky/fun EDH build made around it
The only thing that I could wish for on this card is thst his shuffle ability said "becomes the target of a spell or ability" so that I don't have to use an actual card to shuffle him back into my library and could instead just use an activated ability.
Still, I want to make this crazy, somehow.

I really like this card. Powerlevelwise it is at worse a 1/1 for two that replaces itself. That in and of itself makes Fblthp, the Lost a good card. While not quite elf level of creature type, legendary is a very solid supertype with all of the Dominaria stuff about. But where this card really gets fun is being able to put it into play from your library. I just love the little puzzle of figuring out how to get it from my library to the battlefield.
As for flavor it is a masterpiece. I think it perfectly captures what I wanted in a Fblthp card.

Every time I read a comment about "Well if this card had card draw/trample/haste/indestructible/hexproof/life gain...", I think "You're missing the point." They're armchair developer comments that fail to take into account the card's role in the greater Limited and Standard environment. No, it may not be as good as whatever card you're comparing it to. There's a reason for that. Not every burn spell is Lightning Bolt, nor does it need to be or should be.

That cost can be reduced by 1 with a training grounds or heartstone to make it instead U: Draw a card.

Deadeye Navigator can be used with Vedalken Dismisser and Future Sight. Put in a Sapphire Medallion and a Training Grounds and you get UU: Draw 4 cards.
Isochron Scepter and Elite Arcanist can be used to cast a targeting spell over and over.
Should of had while searching your library you can cast Fblthp, the lost from your library just like Panglacial Wurm or just be a blue version of Norin the Wary
Or maybe something like if it entered the battlefield X times you win the game
