Cite an example, you need a triggered ability that can be activated more than once off a single trigger. When there's a ruling on Haazda Snare Squad that clearly states you get to do this exactly once per trigger.

No, it cannot be "activated" more than once per attack. It's not an activated ability, but a triggered ability that allows you to pay a cost to have something happen. If you can attack more than once, it will trigger more than once, but it only does so once per attack.
Strionic Resonator would work, but I believe it would bump the cost of reanimating two things up to 6 mana (2 x 2 reanimations + 2 for Resonator).
EDIT: I am not at all sure about my second statement about costs here.
Flavorful card though. Besides pseudo-card advantage, I'm sure there are low power creatures you could abuse coming back. Rotting Mastodon for example would be worth B/B if you had it.

Hard to say what impact it will have but certainly a powerful effect. Too bad its same cost on the curve as Rabblemaster though.

Based on existing rulings, yes, you'd need to pay 2 to make the copy, then when the copy resolves you'd need to 'choose' to pay the ability's cost to revive the second creature. There are reasons to do it, but it would be a bit more cost for it.
Man, every Kaalia EDH deck in my meta is going to take this every day and twice on Sundays.

I glad they finally realised that costing huge amounts and having huge impact isn't what everyone wants in a commander, this cycle is more intricate and subtle and there for interesting.

Guys don't laugh. I was brainstorming for good 2 power creatures and I had an earthshattering revalation. This guy, he combo's with norin the wary. Think about it. If you reanimate Norin he comes into play attacking no running away or anything. Finally Norin can show the haters he was a certified badass all along. Oh did I mention Norin is a warrior for that tribal synergy?

This buys back Mardu Heart-Piercer in Limited while triggering Raid. Yikes.
There's a lot to like about this card. Standard is full of good 2 power creatures. The amount of additional value this can create with even one attack is very impressive. Like a lot of good three drops in the format, it can be very punishing if your opponent is unable to deal with it quickly, more so if they spent removal on your two drop.
All in all the leader cycle is actually pretty good. Daghatar is likely the worst card for Standard given you can do better at 4 mana. The others all jam in different ways with other good cards. Competition is going to be fierce, but this card and a few of the others could see some degree of play.
While this card is certainly great, I still think that the best ones are Tasigur and Shu Yun. Tasigur can just get so cheap and Shu Yun+Lightning Strike clearing a blocker allows you to swing for 8.
Just once, for the sake of humor, I want WotC to print a creature with "If 3 damage would be dealt to this creature by an instant, prevent that damage" and flavor text "Still dies to Doomblade"
If we get another un-set I bet this type of thing would be a reality.
another fun legend, like Shu Yu it appeals to my "play a losing strategy that can sometimes win in style". actually it can be used for warrior tribal or for (unreliable) reanimator shenanigans
If she bring back a raid creature will the raid trigger or does she need to have attacked and dealt damage first?
She already HAS attacked, or you wouldn't be bringing that creature back in the first place. So yes, Raid abilities will trigger, because they only care that you declared at attack with something during your Declare Attackers sub-phase.
