They've already shortened the names of Legendary creatures, there is no realistic reason to think they wouldn't do it for planeswalkers too.
The Face Down tokens "issue": Tokens can 100% be face down. Just ask my token copy of Mischievous Quanar. (Or, you know... rule 707.2a. If a face-up permanent is turned face down by a spell or ability, it becomes a 2/2 face-down creature with no text, no name, no subtypes, and no mana cost.)
The no reminder text "issue": They don't generally like reminder text on the face of the game. (Planeswalkers)
Sarkhan 3.0 is quite literally the only walker ever printed with reminder text, and that's because the vast majority of players would not notice the fact that he wasn't a planeswalker anymore without it, since type changing effects are misunderstood more often then not as it is.
The misplaced colon issue:
This is the only actual issue with the card. That being said, it's a major one. It *could* be a mock up, but... considering how... silly... the abilities are, it's not super likely to be real.

Is the opinion of someone on the internet with no evidence to back it up. until we get an actual artist credit, we can't confirm its actually not Swanland's work.

I'm not convinced this is the case. For example, transform creatures are permanents, but they can't be turned face down. However, there is no special rule regarding transform cards being turned face down in this rule. Additionally, 702.25 (the phasing rules) also uses the permanents language and says that permanents can exist while phased out. But tokens cannot be phased out, or at least they immediately cease to exist as a state based action when they phase out.
