When a permanent is turned face down it will be a 2/2 creature with no name, no type, and no color. If the face side has morph you can pay that cost to turn it face up again. However if it does not it will be stuck like that until an effect turns it face up. This is an ability we have seen before.
Colorless nonartifact cards always come first in set order, so Ugin being 003 means there's at least 2 other colorless cards. Karn Liberated has a similar frame. The abilities are out there but Ghostfire as a + is pretty powerful, and turning any permanent face-down lets him kill anything over two turns and still go up in loyalty. Compared to karn his - and ultimate are slightly weaker but his + is much, much stronger. Of course, all this assumes it isn't a hoax.
Interesting, assuming this isn't fake, that means there's at least 2 other colorless cards that aren't artifacts or lands. Honestly I feel like the collector's number lends credence to it being genuine. I'd completely forgotten that colorless nonartifacts nonlands came first in numbering, so if its fake its at least a thorough fake. Hmmm somebody on the reddit thread made a good point, planeswalkers basically always use the full name rather than a shortened one. It should be Ugin, the Spirit Dragon deals 3 damage.
Fake by miles. The set of abilities, on the whole, makes zero sense, but please notice how the colons in front of the cost of loyalty abilities are not aligned with the symbol (unlike all the other existing planeswalkers).
EDIT: Also, it should spell out Ugin's complete name in the first ability and it would likely have reminder text in the 2nd ability. So yeah, it's likely fake.
That's some Ultimate.
According to the gatherer there are currently 29 different planeswalkers. (Ajani, Ashiok, Chandra, Dack, Daretti, Domri, Elspeth, Freyalise, Garruk, Gideon, Jace, Karn, Kiora, Koth, Liliana, Nahiri, Nicol Bolas, Nissa, Ob Nixilix, Ral, Sarkhan, Sorin, Tamiyo, Teferi, Tezzeret, Tibalt, Venser, Vraska, and Xenagos)
I'm guessing that it is real simply because the ultimate is a great marketing ploy to get people to buy a lot of planeswalkers.
Edit: I just look at the colons that mrbrightside mentioned. They do appear to be out of place. For comparison I looked at Dack who has the same text pattern 3,1,2
and I'm revising my opinion to fake.

Still, it seems like a very fair thing for a Planeswalker to do, even if it is just Karn / Garruck 2.0.
I know this is fake, because oh man is this fake, but... I kinda like the concept of this ultimate given the rules of 'outside of the game that you own' in official tournaments is limited to your sideboard.
Meaning to get the most impact from his Ultimate, you have to put 15 of your favorite (legal in the format) walkers in your sideboard, meaning you don't have a conventional sideboard for poor match-ups.
Ability-wise, this card seems reasonable for Ugin.
But wording-wise, it feels off. Like mentioned before, the card would most likely use "Ugin, the Spirit Dragon deals 3 damage to target creature or player." There is definitely space for the entire line.
In addition, I think that WotC would put reminder text that a face-down permanent is always a 2/2 creature. Yes, it's an ability on a Mythic-Rarity card in a regular expansion, but I just feel that the face-down rule is one of the more "obscure" rules that WotC would put reminder text for (For example: Ixidron). Like the first ability, there is more than enough space for the reminder text.
