The one we know of is Teferi, obviously. But what's interesting is that we're getting 1 brand new walker and one we've met but don't know. My guess is this could be the Lithomancer that helped seal the Eldrazi that helped seal the Eldrazi along with Sorin and Ugin, as it would tie thematically with the current block, but who knows at this point. Sorry, don't mean to speculate too much, just sharing the article

Actually, Ob Nixilis lost his spark when Nahiri (The Lithomancer) planted a hedron in his head.
While we do know her name, she was never officially confirmed to be The Lithomancer, but with her powers over the hedrons and Sorin mentioning her alongside Ugin, it makes it pretty obvious she is.
There's also no way a kor planeswalker with mastery over binding magic is not white, so Nahiri being in would mean no Serra...
I'm still very happy we are getting a control-based white planeswalker, but that was the best opportunity for a Serra card...
You don't call "dying to removal" if the removal is more expensive in resources than the creature. If you have to spend BG (Abrupt Decay), or W + basic land (PtE) to remove a 1G, that is not "dying to removal". Strictly speaking Goyf dies to removal, but actually your removal is dying to Goyf.

It's the basest of baseless speculation, but Angelic Field Marshal is an Angel and is ostensibly meant to represent a lieutenant to the commander... who better than Serra? I know it's silly to be drawing conclusions based on "white has Angels," but it's at least cause for hope.
Besides, with Sorin grabbing Ugin in Khans block, and with all signs pointing to an imminent return to Zendikar, I imagine they save the Lithomancer for an expert set.
I don't believe White will be Serra just based on the fact that the new Field Marshal is an angel. It said originally they were going to use Ajani as the walker and if you look at the designer for the white deck, it says he loves cat tribal. Not sure if there is another White Cat PW other than Ajani, but anyone know of any legendary creatures that might be made for the deck?
I wish my 4/4s with haste and first strike that acted as Goblin Warchiefs were better and didn't die to instants and overshadowed all other 4 drops in the format. And that they had evasion too, and some way to dodge wrath effects.
I really want to believe that Serra is the white walker, but I can't think of any other planeswalker that we know, but not by name.
The only reason I could think to not pick Nahiri is, due to her importance in the Eldrazi story, they might be willing to print her on a standard set. But then again, they could print her now either to bring up attention about her role in the plot or because she won't take part on the next events concerning the Eldrazi, so now is good moment to print her.
About the other walkers/legendary creatures, I think they will try to cover as much nostalgia as they can.
-Tarkir and Theros might not get anything because they are current planes, so nostalgia on them can wait
-Ravnica probably won't be there either as de dual-color thing is so strong in the plane that a monocolor guy wouldn't feel as "Ravnician" as it should
-Alara, for the same reasons, doesn't seems like a good pick
-Innistrad got two legendary creatures
So we have:
-Zendikar: Speculation are leading to believe we'll get two walkers here: Ob Nixilis and Nahiri/Lithomancer. Either of them seems reasonable and I guess it is safe to assume Zendikar will get some nostalgia-love in this, as it has been a time we've been there, is a popular plan and what happened there is still a central part of current story.
-Mirrodin/New Phyrexia: Also a popular plan that is still relevant to the story. I can only think about two walkers here: Slobad and Jeska. Also Jeska seems like a perfect choice for red because:
1-NOT CHANDRA
2-She is relevant to at least FOUR blocks: Odissey, Onslaught (as Phage), Mirrodin and Time Spiral
3-STILL NOT CHANDRA
4-Though I guess Phage is more popular than Jeska herself, it would still bring up instant "Thanks for that, I love you Wizards" for giving her a PW card
5-Also, she has more design space than Jaya to feel a walker apart from Chandra
-Kamigawa and Lorwyn: Both of them stands among less popular planes (at least considering the Modern ones). I could see them getting a Legendary Creature, specially if they intend to never revisit these planes (as seems to be the case in Kamigawa). Something like the presence of Tamyo in Innistrad "We're not getting back to this plane, so have this tiny bit of nostalgia, cause that's all you'll get".
-Dominaria: Here's where the majority of options lies. We already got two cards, and I'm pretty sure we'll get a few more.
-At least one should be Otaria-relevant (as I said, Jeska would be a perfect option).
-We already have Teferi, that is the face-guy from Time Spiral with important roles on previous blocks.
-Some of the Nine Titans, or any Invasion-relevant character would be a nice choice too. Maybe someone from the Weatherlight crew, what would cover even more blocks, but I'm not sure who could it be (Squee maybe?)
-Finally, we have those really old guys like Feldon that could show up. But as we already got him, I think we can get only one more in this category (a really old character that hasn't showed up for a long while).
-Ulgrotha/Mercadia/Other old-and-not-so-popular-planes: I see them as I see Kamigawa/Lorwyn, but with fewer chances to get a card. Maybe one Legendary creature to say "Hey, this existed once upon a time". But I wouldn't put my two cents on it.

And it is exactly because we oficially don't know that Nahiri is indeed the Lithomancer that she she falls in the category "A planeswalker we know, but not by name". The hints leading to it are pretty strong though.

If you're referring to New Phyrexia, we know who made it. Karn Not certain about the original Phyrexia.
