I'm going to propose something crazy. Have you ever noticed how Kozilek and Ulamogg share the same basic body shape, plus or minus a few arms, while Emrakul looks completely different? With Kozi and Ula dead, there's a chance we see all new Eldrazi Titans. But how? From where?
What if Emrakul is the key to the Eldrazi Trinity? For instance, she comes to a plane and creates or "selects" her two cohorts? She is the Eldrazi known for altering living things, after all. Given this, killing Ula and Kozi was almost pointless, besides saving Zendikar. Upon arriving on a new plane, Emrakul could just "select" her new trinity from inhabitants of the plane she's in.
Later down the line that could lead us to ask, "Who were Kozi and Ula?"
Of course, it is possible they just bring back Ula and Kozi and my idea is a bust. In that case, I'm calling it: they're going to defeat the Titans by trapping them in the moon.

So what if sorin bound Nahiri on zendikar so that she'd be in stasis with the eldrazi and not age so that she could pretty much be tied to defending the multiverse from them (a riff off the nahiri trapped in the hel vault theory). She gets out, is pissed, and was affected similar to tazri by kozilek the whole time. So now she's going to make these inter planar mana beacons for Them to come consume specific mana rich (or broken? Or whatever their deal is) planes. She's still angry at sorin (maybe kozilek's influence prayed off her desire to harm a specific person she doesn't have fond feelings for).
She goes to innistrad first because screw sorin, that's why. So the in-fighting and madness is due to her realigning the planes mana to serve two purposes, reduce the planets defense system and hide her trail. Jace ends up digging for all these lose ends and finally figured out emrakul is just being called over after its too late to stop it.
There wouldn't necessarily be a physical manifestation of eldrazi if the feeding has only just begun maybe? Not sure what the time frame is on that.
The art book stated that emrakul distorts biology (hence the skinless wolf with 4 eyes, the youths with disformaties, etc.). Just the wind kind of flies in the face of this theory but there are plenty of mutated humans in the artwork so far. So I think there's more evidence then not.
Eldritch moon could be the answer for them trapping emrakul, I.e. Jace realigns the hedron network with tamiyo and Lilliana's help and sticks Emmy in there for "safe keeping."
Innistrad is safe, for now (ooo spooky plot hook for the next horror sequel), and everyone calms the junk down with Sigarda as head angel and founding a new safe haven for humans.

They purposely made the flavor text different on the journal for each language, and the number game with the clue tokens works in every language... to give the flavor text from a different card pertaining to the characters involved in this story... It is more likely you crap out a baby today than that all happening randomly.

I love that idea. It even fits with the three plot-threads we seem to have in SOI:
1) The mutations and emergence of organic horrors
2) Nahiri's corruption of Avacyn (presumably through the cluestones)
3) Whatever is stirring in the water
If you line it up like that you even get a similar setup as with the original titans. Nahiri has power over stone and Kozilek was the titan embodying land, earth and rock. And the diety/monster of the sea would become Ulamog 2.0, the titan that represented the sea.
The more research I do, the more evidence I find for Emrakul being the progenitor. Just look at the Emrakul Wiki and the effects she has on living organisms. Even if Emmy isn't on Innistrad, I'm sticking with her as the reason there are three Titans.
Since Tarkir we have an Eldrazi related theme in Magic, I do not doubt that this block will be about it too, as it seems to be the major plot right now. BUT it is more likely that the Eldrazi theme will be rather subtle, like in Tarkir. It makes much more sense for me to leave the Eldrazi titan Emrakul as a roaming threat in the multiverse, not throwing her in right away.
I think she will get a whole double block with own game mechanics and everything. It would be cynical to introduce the strongest titan in the second half of Innistrad and probably kill her quicker than the other two.
It's an interesting idea that Emrakul could be the originator of all Eldrazi as the Eldrazi mother. That Ula and Kozilek may have once been corrupted plane-bound entities that grew enough into their Eldrazi forms to leave the plane(s) on which Emrakul first corrupted them, and traveled with her to other worlds, eventually becoming titans in their own right. That they could be members of her lineage after all. Who knows, but it's a plausible idea.

The engine however is Emmy Bolas is set on getting his power back and he has been known to manipulate and decieve well. At this point, I can't say with 100% that it's Eldrazi involvement, because of the eyes. Maybe Innistrad is affecting the Eldrazi in different ways, or vice-versa. Eyes just haven't been a defining feature of Eldrazi.
Eyes in weird places actually are a defining feature of Kozilek's brood. But, considering that we are discussing Emrakul, that makes it even more of an argument against her. It would be weird if they had so similar specific symptoms.
Maybe Kozilek trick himself out of the scene, escaped, an only 'indestructable' Ulamog got whacked.

But isn't that also an argument against Emrakul - like how would Nahiri even know of the Eldrazi escape, being absent for 1000. It doesn't really make sense that she'd randomly try to lure the Eldrazi to Innistad, without knowing of them being freed. And how would she?

She recieved a call from Sorin via the Eye of Ugin when the last outbreak happened. So she definitely knew that something was going on, and she might very well just drop by and take a look. Although she might not know that the other two were destroyed and maybe she considers Zendikar doomed, hence all that hate.
Everyone has seen that symbol that the culists are using right? What if the circle is the blind eternities and the three tentacle-like objects are the three Titans?
And I thought I was special... I thought that maybe emrakul had created the other two a few days ago, and I wasn't the only one.

As time goes on, new titans could become more individualized, losing the trademark latticework that Emrakul's spawn has and developing new features and spawn unique broods. If Brisela lives through the rest of the set, we might be seeing the birth of a new titan.
