I believe people are erring when they assume that demons(and possibly angels) reform as the same being after death.
I was under the impression that it had been stated/implied in original Innistrad that when they died their essence(mana, whatever) returned to the fabric of the plane, where it was then available for the formation of NEW demons. Not a technical rebirth of the old ones.
This would mean that while we might get A b/w angel should there be an Innistrad3.0, it wouldn't be THE b/w sister.

This is correct. When the demons for they have a vague idea of who they were before but they are new demons. For example many believe that Ormendahl is Grisselbrand reincarnated.
Yeah I don't really think Ormendahl is Gris either.
Also Ulamog was able to create Vampires on Zendikar while they were sealed away and their presence caused Zendikar to create the Roil in response. So no the presence of the Eldrazi was not negligible.
Ormendahl is not Grieslbrand. The art book confirmed this.

The planeshift guide furthers confirms this. Ormendahl survived under the cathedral catacombs, implying that he existed when Avacyn retruend and began purging the demons.
Don't know why you're all putting so much faith in the fourth sister. Her story is only mentioned to describe Avacyn's personality as a hard ***** and demonstrate her power in comparison to the other angels. The fourth sister as a character is totally irrelevant to the story.
