I have now added Ugin to this list, since it is now known that he is native to Tarkir, and he is all but certain to appear at some point during the Tarkir block.

I wouldn't immediately assume Ugin is from Tarkir from what we know. Sure, back then when we saw Markov Patrician, some of us probably assumed Sorin was from Innistrad. It was indeed proven true later, but before then there was still the possibility that he came from somewhere else, then sired a vampire family in Innistrad, no?
Currently the only certain thing we know of is that Ugin died in Tarkir, but not necessarily was born there, unless the Wiki was out-dated by a few minutes

Isn't it possible that Otaria has managed to rebuild itself by now? I mean, it wasn't hit by the Invasion which was the cause of most of the destruction throughout the rest of Dominaria. Once the rift caused by Karona was closed, it should have been able to start healing and rebuilding, should it not? Or am I missing something?
There literally wasn't any life left on the continent. It kind of wouldn't have mattered if it started healing or not, there was nothing there left to rebuild anyways.
Are you talking just Otaria or the whole of Dominaria? I don't remember what happened on Otaria but I'm sure it would have been repopulated over time.

People would have to migrate back to Otaria to repopulate it, and to be honest, it wouldn't be worth it. There was literally no life left. The soil itself was depleted and saltblasted and Otaria itself is so isolated a continent that the Phyrexians didn't even manage to invade it during the Invasion.
Now that new planeswalker cards have been released in Commander 2014, I have updated this list to include them.

Those are Dominarian locations, but you had gone through the effort to specify, so we thought we should narrow it down further for you.
As to why Freyalise is associated with Llanowar, it's because she set herself up as a goddess of the elves and basically conquered them for her own vanity.

If you try to make something like this list for the PW characters., you shall at least read the wiki entries at our wiki. Much is answered there.
As for the formulation "she helped elves from Fyndhorn and Llanowar to survive the Ice Age", that is used for her PW vignette, I have a little problem with that. Llanowar was canonically made what it is around Weatherlight, and we do not know whether and how it existed during Ice Age.

My Gripe with all of this is that all of MTG's major planes (the ones that got card sets) are accounted for with planeswalkers, except for one.
I'm wondering when we're going to see a PW from Lorwyn/Shadowmoor.
I can imagine a Kithkin PW with some unique thoughtweft abilities. or a weird treefolk PW popping up on other planes freaking people out. we need to make this happen.

I don't think that...
A) ...the flamekin are exactly artificial beings. We have never been told explicitly that the flamekin are created in the same vein as Angels and Demons, and infact we were told that there is a major difference between the flamekin from Lorwyn and the Flame-Kin from Ravnica. Frankly, we do not know the exact process of how flamekin are created, it has only been hinted at.
B) ...not being "born" in the conventional way precludes a sapient being from becoming a planeswalker. I mean, Ugin is a spirit dragon native to Tarkir and we know that the dragons on Tarkir are generated by elemental storms. So unless we assume that there was an immensely convoluted reason out of nowhere that Ugin shouldn't be able to have a spark but does, somehow, then I think having flamekin planeswalkers is perfectly within the realms of possibility.

If they aren't spiritually complete without finishing their pilgrimage, it makes it a lot harder for the spark to be believably bestowed by chance to them.
I am probably every color-combination it is possible to be, though it's really hard to figure out what it would mean to be 4-colored....it doesn't seem logical to be 4-colored without being 5-colored.

Lorwyn is sparkblind. They have never, in their entire history, heard of or developed any legends about planeswalkers. That speaks really poorly for their chances.
