Yes, we first saw Sarkhan on Jund and Elspeth on Bant; however, they were not born there. Sarkhan came from a plane where dragons were extinct and where warlords constantly fought, while Elspeth came from a plane dominated by Phyrexians.

Isn't Nicol Bolas from Dominaria, too? He's related to the other Elder Dragons who all live on Dominaria, so unless some Planeswalker brought them all to Dominaria, it seems a safe assumption. Depending on where you go in the Wiki, it's unclear. Or did the Elder Dragons war span planes without them actually being walkers?

Anyways, the Elder Dragon empire was something that stretched across multiple planes, dozens if not hundreds of them. At their peak, they were comparable to oldwalkers in power and they obviously had some way to get from one world to another, but we never really learned much about that.
I am fairly sure that Garruk is from Shandalar. At the very least I remember reading somewhere that his axe and helmet are from there.
His helmet was taken from the man who killed his father. No idea about the axe; he didn't have it when he escaped to the wilds and had it when he returned.

The wurms and baloth in the Wild Son look distinctly Otarian, but that seems unlikely what with Otaria being a ruined wasteland.
Isn't it possible that Otaria has managed to rebuild itself by now? I mean, it wasn't hit by the Invasion which was the cause of most of the destruction throughout the rest of Dominaria. Once the rift caused by Karona was closed, it should have been able to start healing and rebuilding, should it not? Or am I missing something?

Liliana first encountered Garruk on Shandalar (since we have subsequently learned that is where she found the Chain Veil), but we have no knowledge as to whether or not that was Garruk's home plane.
Also, I believe it was established that Liliana had been to Shandalar prior to being sent there to retrieve the Veil. Depending on who or what the Raven Man turns out to actually be, it is remotely possible that Liliana is native to Shandalar (she encountered the Raven Man first on her home plane and then later on Shandalar; if the Raven Man is not a planeswalker, that could be evidence those two planes are the same).
Well, we may not know his birth plane, but I swear I read something once that said the Jace had claimed Ravnica as his home long before RTR. So on a minor technicality, his home is Ravnica. Plus with him being the Living Guildpact, he pretty much is stuck there. He can leave, but not for long periods of time.
