Ok, I'm pretty sure I have this right, but the comprehensive rules on this are a little muddled:
If my opponent attacks me (player) with Tel-Jilad Fallen I take 3 damage as well as a -1/-1 poison counter, correct. Same applies if I block or get blocked by this creature, true? Thanks!
You are incorrect. Infect replaces combat damage, so if it connects, you take no damage but 3 poison counters. If you block, the blocking creature will take no damage and 3 -1/-1 counters. This is of course assuming no change in power.
Note that damage has to still occur for infect to work, so damage prevention (fog, protection from green ect)will still stop the counters.
Aha!!! So the '3' in the (3/1) portion of the card is the multiplier for how many -1/-1 poison counters are applied. Thank you! I would have never thought of it that way.
Dealing damage and what that damage results in are two different things. You can be dealt damage without any results showing for it, and you would still have been dealt damage (a scenario for that can be constructed, but usually damage does show some sort of result).
119.4. Damage is processed in a three-part sequence.
119.4a First, damage is dealt, as modified by replacement and prevention effects that interact with
damage. (See rule 614, “Replacement Effects,” and rule 615, “Prevention Effects.”) Abilities
that trigger when damage is dealt trigger now and wait to be put on the stack.
119.4b Next, damage that’s been dealt is processed into its results, as modified by replacement
effects that interact with those results (such as life loss or counters).
119.4c Finally, the damage event occurs.
The possible results are:
119.3. Damage may have one or more of the following results, depending on whether the recipient of
the damage is a player or permanent, the characteristics of the damage’s source, and the
characteristics of the damage’s recipient (if it’s a permanent).

Platinum Emperion and getting attacked by someone's Commander is probably the most relevant. Your life total doesn't change, but you were still dealt Commander damage, and taking 21 Commander damage from the same Commander still makes you lose the game.
@jason33w: Your interpretation is basically correct for the old creatures that doled out poison counters, eg Sabertooth Cobra. Those creatures deal combat damage as normal, and have an ability to also give some set number of poison counters which is independent of the amount of damage dealt.

This is incorrect. Infect doesn't replace damage. Damage is still dealt. It just changes the results of the damage to poison counters (player) or -1/-1 counters (creature).
