If I have my Mimeoplasm out as a copy of Creature A with counters from Creature B and I Supplant Form it, does the copy come in as the same, does it trigger again (allowing me to choose two new creatures to exile) or does it come in as a 0/0 and die?
When you copy an object you take into account any copy effects affecting that object. So the token you get from Supplant Form will be a copy of whatever The Mimeoplasm was copying (i.e. Creature A), without any extra +1/+1 counters.
Dilithium got it right. Lets say you cast The Mimeoplasm and exile a Siege Rhino and an Anafenza, The Foremost. You choose to make it a copy of Siege Rhino and then take the four +1/+1 Counters from Anafenza's P/T. What you now have on the field is a Mimeoplasm that is a copy of Siege Rhino with four +1/+1 counters on it.
If you now cast Supplant Form and target your Siege Rhino(Mimeoplasm) you will get a copy of Siege Rhino, because that is what Mimeoplasm is. When you make a copy using some spell or ability, you basically put your card on to a copy machine and just make an exact copy of it.
