Nope, it doesn't. The Crusade has a triggered ability. It uses the stack and can be responded to. Mikaeus would not get the counter in time to live, since it would die due to state-based actions before the Crusade's trigger can even go on the stack.
Just to be clear: When Natedogg speaks of "the crusade" he is presumably talking about Cathars' Crusade not Crusade. (Yes, this can be inferred from context, but given the similarity in names it pays to be extra clear.)

I know that Mikaeus, the Lunarch may enter the battlefield and survive with Crusade in play, regardless of +1/+1 counters.
Does the same hold true for Cathars' Crusade, or do state-based actions take effect before the counter's added?
