The Equip ability targets, and Auras target when they're cast as spells, so the answer is yes unless there's an effect attaching that equipment or aura without actually targeting (Quest for the Holy Relic, Replenish, etc.).
The equip ability targets, and aura spells on the stack target. Either of those will cause the illusion's sacrifice ability to trigger and be sacrificed before the equip ability or aura spell will be countered by game rules for lack of legal targets.

Normally, yes. While an Aura or Equipment attached to a creature doesn't target by default, Aura spells and the equip ability do. So the only way to attach an Aura or Equipment to a creature would be something along the lines of Stonehewer Giant or Autumn-Tail that does so without targeting.
Just need to verify, when Phantasmal Image Is equpipted or enchanted (by control magic or something) , will it auto sack?
Yes if it is the target of any equip ability or any auras it will trigger the sacrifice me ability. If the equipment/aura is put on the creature by some means other that being targetted it will survive.

Yes for equipment and for when an enchantment is cast targeting Phantasmal Image. However, if you were to have an aura enter the battlefield without beig cast (for example, having Phantasmal Image attack alone with Sovereigns of Lost Alara), it will not target and Phantasmal Image will not be sacrificed.

Espeically clearing up the differnce between attaching and equipments...
Had Phantasmal Image on a mimic Vat, was wondring if it can be abused with equipments.
