I'm on the play and I have 7 cards in my hand, one of which is Serum Powder. I don't like my hand, so I exile my hand and draw a new 7 cards. At this point, do I immediately again have to make the mulligan decision or does my opponent have to choose what he is going to do first?
My understanding is that if you use Serum Powder, you immediately make your decision of whether to keep, mulligan, or use (a newly drawn) Serum Powder at that point.
After using Serum Powder's ability, you still get your chance to make the actual mulligan decision.
103.4a If an effect allows a player to perform an action “any time [that player] could mulligan,” the player may perform that action at a time he or she would declare whether or not he or she will take a mulligan. This need not be in the first round of mulligans. Other players may have already made their mulligan declarations by the time the player has the option to perform this action. If the player performs the action, he or she then declares whether or not he or she will take a mulligan.

Both players have to make their decision of keep/mulligan, in turn order, but before any mulligans are taken.
In this example you have yet to make your first decision, (since the powders don't count as mulligans), so you will not know if your opponent is keeping, since you are going first.
