If i attack with a creature using Venom or Dead-Iron Sledge, and I regenerate my creature, do the blocking creatures still die even though regenerate removed my creature from combat? Also same question but what if defending player has first strike?
Yes, they will still be destroyed, because the triggered abilities only care if the creature was blocked, not if its still in combat or even in play at the end of combat. This is called a delayed trigger, because the condition is met when the blocks are declared but the effect does not resolve until later.
Note that with Dead-Iron Sledge you may need to regenerate your creature twice (Toski not withstanding). If your equipped creature is dealt lethal combat damage and you regenerate it, Dead-Iron Sledge will try to destroy it again after combat, making a second regeneration necessary.
When a creature enchanted by Venom is declared as blocker or a blocker is declared for it, a triggered ability goes off. That trigger will resolve in the declare blockers step and only sets up a delayed trigger for the end of combat step. What happens to the enchanted creature after the first trigger resolves has no impact on the delayed trigger, that one will destroy the other creature in the end of combat step. It can be responded to, however, being a trigger itself. You will have to regenerate your creature before combat damage is dealt (if that would be lethal).

If the equipped creature regenerates to avoid dying, it is removed from combat when the sledge's trigger resolves, and it won't be involved in combat damage, neither dealing nor receiving. So one regeneration is enough.
