Yes. A creature with damage equal to or greater than its toughness marked on it is destroyed when state-based actions are checked, regardless of what its toughness was when that damage was dealt.

No, when a creature dies due to having 0 or less toughness when SBAs are checked, it's not destroyed, it's just put into its owner's graveyard. This is why a Tragic Slip will kill a Blightsteel Colossus.

Yes, a combination of toughness reduction and damage such that Lethal Damage is marked on the creature the creature is 'destroyed'. However complete toughness reduction is not destruction but simply moving to the graveyard.

From having 0 toughness. It's not destroyed, it's simply put into its owner's graveyard; see rules 704.5f-h. The gist is that only creatures with toughness greater than 0 can be destroyed due to having lethal damage marked.
