Demanding Dragon's triggered ability means, in part, "... target opponent may sacrifice a creature. If they don't, [Demanding Dragon] deals 5 damage to that player" (C.R. 117.12a); thus sacrificing a creature this way is optional (C.R. 603.5), but damage is dealt if none is sacrificed this way (C.R. 117.12). The opponent does not have to sacrifice a creature. They are offered the choice to sacrifice a creature, if they choose not to then the will be dealt 5 damage.
If the targeted opponent controls a creature, they can choose whether or not to sacrifice. If they control no creatures, they have no option but to not sacrifice (having nothing to perform that action with) and thus must take the damage.
