Does the interaction between Polyraptor and Aether Flash create an unbreakable loop game state? Does this mean the game wouldn't ever end if the combo is uninterrupted? If there are no wincons on the board (Purphoros, God of the Forge, etc.) what would happen?

Since neither trigger is optional, the loop would be infinite without an external factor to break it or end the game. When such loops occur, the game ends in a draw, but the tournament rules have measures in place to punish players to who initiate them intentionally.
Ew, so unless I wanna be "that guy" I would have to find a way to either win because of it or find a way to end it? Would the same happen if Forerunner of the Empire was in play (and had indestructible and/or couldn't receive damage) and Polyraptor entering the battlefield afterwards?

Forerunner of the Empire has an optional trigger, meaning, you can choose to have it deal damage or not. And because of this, the loop with the Forerunner will end (the controller has to choose not to deal damage with he trigger at some point), the game will not be a draw.

There are no tournament rules against instantiating a loop that draws the game. You may be confusing this with the rules against iterating a loop without a defined number of iterations "until" it reaches a defined end state (i.e. the "Four Horsemen" combo with Mesmeric Orb + Basalt Monolith with a library containing an Eldrazi titan but also some number of cards that need to be in your graveyard at the same time in order to win).

I'd like to add to this question a little... I hope that's okay. If I had Impact Tremors on the battlefield along with Aether Flash and then I played Polyraptor, would the damage be dealt to opponents, or would that not happen because the game is stuck in the unbreakable loop?
Every time a token is created, you will have two triggers; one to deal damage to the token and another to deal damage to each opponent. Eventually, that 1 damage will (presumably) kill your opponents. If an opponent has something that prevents them from losing (like Platinum angel) the loop will continue forever and the game will be a draw.
As long as you can kill everyone with the loop, or you can stop it somehow, it is not a draw.
