1) "Create" is a rather new term for putting tokens on the battlefield. So there really is no difference, just a change in terminology. It's shorter, and thus leaves more room for other rules text on the card. And it's arguably more natural speech.

If Breya and Confusion are controlled by the same player, that player can choose the relative order of the triggers on the stack, so he can choose to put Confusin on first, then the token creating. The token creating trigger is controlled by the player under whose control Breya entered the battlefield, so even if Breya changes control before the tokens are created, the tokens enter the battlefield under the original controller's control. Each token will cause another trigger of Confusion, so there will be two Confusion triggers to go on the stack at the same time. The Confusion's controller chooses the relative order of those on the stack, while the tokens' controller chooses their targets (which can be noncreature artifacts, due to the tokens' double card type). When each trigger resolves, control of the associated token and the targeted creature/artifact is exchanged (if they are still controlled by different players).
