After swapping around my various flip cards through several different decks, I was curious about a potential mistake I can see myself making in the future. I register for a competitive REL event and fill out a decklist, registering 2 copies of search for azcanta and no other flip cards. In my deck I am using checklist cards and have the proper copies in my deck box. Then during a game I draw a checklist card and find that I didn't mark off which card it was supposed to be.
Assuming that everything else is "as it should be," as in I announced the card I registered, took it out of my box and put it into play and so forth with no other flip cards in my possession what is the official ruling in this situation?

Simply put, a checklist card without an item checked is not a legal card to have in your deck in a tournament. When that problem is found out, how it is adressed will depend on the rules enforcement level (REL) and the circumstances. In the situation you describe, on Regular REL, I would just have you identify your checklist cards clearly as Search for Azacanta, randomize your library if needed, and move on, but on higher RELs, in addition to that, an official warning, or possibly a game loss, for deck error (see the Infraction Procedure Guide, section 3.5) will be in order.
I'm a former judge (lapsed), who keeps up to date on rules and policy. Keep in mind that judges' answers aren't necessarily more valid than those of people who aren't judges; what matters is we can quote the rules to back up our answers. When in doubt, ask for such quotes.
