Yes; if you cast a spell without paying its mana cost and you pay no mana to do so, you must also pay an additional 3 because Trinisphere doesn't even look at nonmana costs and its effect is applied after all other cost increases and reductions.
Yes. Alternative costs only replace the mana cost, but additional costs, cost increasers, cost reducers, and Trinisphere all apply after that. And Trnisphere is always last as it doesn't fit any of the other categories. You always calculate the cost of a spell this way:
