1. Yes. Even when spells and abilities instruct you to draw multiple cards, you still draw them one at a time, which allows you to reveal a Miracle card if it's the first card drawn in a turn.
2. Yes. Keep in mind that casting it on your turn during your draw step is also a time when you normally can't cast sorceries, either. Casting a spell with its Miracle trigger ignores ordinary restrictions on when you can cast sorceries.
3. Yes. Miracle is a triggered ability, so you can cast a discard instant while it is on the stack.

If you didn't draw any other card this turn, then yes, you can. Notice you'll reveal Entreat before drawing the other two cards, but will get to cast it only after putting two cards from your hand back in the library (and of course, if you put Entreat back on the library, you won't be able to cast it)
