If all three of these are in play do you lose life for each card? Or will megram go back to your hand thus only taking damage for the first card discarded

Megrim will trigger for each card an opponent discards due to Whispering Madness. The first trigger to resolve will cause Dissipation Field to trigger, the trigger is put on top of the stack and resolves next, putting Megrim into its owner's hand. But all the other Megrim triggers will still resolve and make it deal damage! Even though the enchantment isn't around anymore.
In general, removal of the source of an ability on the stack will not stop that ability from resolving.

Note, that Megrim and Dissipation Field have to be controlled by different players, who are opponents, in order to trigger both. Because Megrim only triggers for opponents discarding cards. If the same player controls both enchantments, Dissipation Field will not trigger, since nothing is dealing damage to its conroller.
No. The discard and draw of Whispering Madness all happens during its resolution, before any of Megrim's triggers even makes it to the stack. They are all put on the stack afterwards, then resolve one at a time, the first one to do so causing Dissipation Field to trigger. And the resolution of that trigger comes way too late, the discard has long since happened.
