Your Blademaster can either get 0, 2, or 4 counters, the choice being simply because the Blademaster's ability has a "you may" clause. Because of Panharmonicon, the ability triggers twice. Each instance resolves separately, and for each, if you opt to put a counter, you put two counters instead of one because of Hardened Scales' replacement effect. So 4 total at most.
Note that Panharmonicon never itself triggers; its ability is a static ability that modifies the rules of the game, not a triggered one.
I'm a former judge (lapsed), who keeps up to date on rules and policy. Keep in mind that judges' answers aren't necessarily more valid than those of people who aren't judges; what matters is we can quote the rules to back up our answers. When in doubt, ask for such quotes.
And your point about the "may" part is important because even as an experienced player I often forget, especially if I have a pile of Allies on the table.

I successfully cast a Kazandu Blademaster.
Does the Kazandu Blademaster get 3 or 4 +1/+1 counters?
1) Kazandu Blademaster ability + Hardened Scales for 2 counters
2) Panharmonicon causes the Kazandu Blademaster + Scales to trigger again for 2 more.

Does the Panharmonicon trigger immediately after KB's ability triggers, thus giving him 2 counters plus one more from Hardened Scales?
I guess another way of asking would be - do I as the controller dictate how all those things go on the stack?
