I assume the replacement effect remains in place and rule 614.6 applies: "If an event is replaced, it never happens. A modified event occurs instead, which may in turn trigger abilities. Note that the modified event may contain instructions that can’t be carried out, in which case the impossible instruction is simply ignored." Is that right?
2. If the replacement effect is in effect, what happens you skip your upkeep and/or entire turn? I assume the replacement continues to apply indefinitely until you have another upkeep.
1. Yes, the ability creates an effect that is independent of the source; the effect still applies after Soul Echo has left the battlefield, and you do as much as possible of the effect, in this case that means replacing the damage with doing nothing (because removing counters is no longer possible).

500.6. When a phase or step begins, any abilities that trigger "at the beginning of" that phase or step are added to the stack.
614.10a. Anything scheduled for a skipped step, phase, or turn won't happen. Anything scheduled for the "next" occurrence of something waits for the first occurrence that isn't skipped. ...
So if you skip your upkeep, the upkeep doesn't begin and the effect won't end.
