If Tephraderm is blocked by a 5/4 creature, both creatures die, correct?
To clarify, Tephraderm's ability doesn't do anything here, Tephraderm could just be any 4/5 without abilities. In the combat damage step, the 5/4 deals 5 damage to the 4/5 and the 4/5 deals 4 damage to the 5/4. They both die from having lethal damage marked on them, then any triggered abilities that triggered from the damage or the deaths go on the stack. Tephraderm's ability goes on the stack and resolves then, but since the 5/4 is already dead, the ability doesn't deal damage to anything.

I'm a former judge (lapsed), who keeps up to date on rules and policy. Keep in mind that judges' answers aren't necessarily more valid than those of people who aren't judges; what matters is we can quote the rules to back up our answers. When in doubt, ask for such quotes.
