so new to magic and been playing it a bit now but still hazy on this madness rule. I understand majority of it but what happens when someone exiles one of my madness creatures. does madness trigger because its gone into exile or does madness only trigger when I discard it from my hand into exile. and what happens if someone makes my exile the top card from my library and its a madness card? been looking for an answer but nobody seems to have asked that yet. maybe its obvious and I just dont see it.
Madness only triggers when you discard a card with Madness. Putting it in Exile is part of Madness, not what triggers it. So exiling a madness creature from the battlefield or exiling a madness card from the top of your library will not trigger Madness since you are not discarding that card in either case.

Those are very old rules. For starters, abilities don't say "Removed from the game" any more, they say "Put into exile". For another, with the recent release of Shadows over Innistrad, discarding into exile is no longer a choice, the madness card is always discarded into exile and then there is an option of whether or not to cast it.
