Is there a difference between combat damage lifelink and lifelink as a result of an ability?
Lifelink is a state-based effect, resultant of any damage to my knowledge.

Yes there is a difference between combat damage. Since the triggered ability is what's causing Nekusar to deal damage, it's the ability that's causing you to gain life as a result of that damage. So you'll lose 1 life instead.

There is a difference, and you will not gain life. Nekusar has lifelink, and his ability is dealing 1 damage to a player, which is modified by lifelink to also cause you to gain 1 life. Rain of Gore sees an ability that is trying to cause you to gain life, and modifies it.
Combat damage, on the other hand, is neither a spell nor an ability. It is a turn-based action, which Rain of Gore doesn't care about.
It's not lifelink that's gaining you the life, per se, it's the damage event that lifelink is modifying that is doing it.
In regards to Rain of Gore Gatherer states:
"4/15/2013 This does not apply to life gain caused by combat damage from a creature with lifelink."

If you mean it would cause you to gain life and therefore would be modified by Rain of Gore into life loss instead, that's correct.
