So Kulrath Knight prevents creatures with counters from attacking. Exalted doesn't get counters UNTIL they attack so what happens? Are they removed from combat?
1) Exalted doesn't give creatures counters upon its resolution. It simply gives them power and toughness bonuses that last until the end of the turn.
2) If a creature somehow gets counters after it's been declared as an attacker or blocker, it won't be removed from combat. That's because Kulrath Knight only prevents creatures from being declared as attackers or blockers before they're actually declared as such, not after.

Cards do what they say they do. No more. No less.
An effect only puts a counter on an object if the effect specifically tells you to put a counter on the object.
A card such as Giant Growth does not put counters on its target. Rather, it creates a static ability that gives it +3/+3 until end of turn. Battlegrowth does put a +1/+1 counter on the creature. It is very clear in that it puts a counter on it.
This is in contrast to something such as Armory of Iroas, which does put a +1/+1 counter on the creature when it attacks (again, it uses the word "counter" in the effect). In this case, the creature can still attack. Once a legal declaration of attackers has been made, adding an effect which would make the creature no longer able to attack does not retroactively undo the attack. Instead, the creature continues attacking as normally. The only time attacking restrictions are checked is at the instant when attackers are declared.

It is not a counter because the text does not say it is a counter. Simple like that.

Notice it does not say "put a +1/+1 counter", just "gets +1/+1".
Contrast with:
702.99a. Evolve is a triggered ability. "Evolve" means "Whenever a creature enters the battlefield under your control, if that creature's power is greater than this creature's power and/or that creature's toughness is greater than this creature's toughness, put a +1/+1 counter on this creature."
The first simply changes the creature's P/T for a duration.
The second gives a counter to the creature, and that counter stays on it until something else removes it. The counter, not the ability's effect directly, changes the creature's P/T for as long as it exists there.
