Its a concept made manifest, as with every elemental its from the minds and actions of the other denizens. That its name is that result. This applies to every elemental within Lorwyn/Shadowmoor.
For this you will want to look at the definitions of "Primal" and "Crux".
Primal
1) Relating to an early stage in evolutionary development; primeval.
2) Essential; fundamental.
3) Original, Primitive
4) First in importance : Primary
Crux
1) A puzzling or difficult problem : an unsolved question.
2) An essential point requiring resolution or resolving an outcome
3) A main or central feature (as of an argument)
Next take what its flavor text talks about: "When nature is backed into a corner, just like its children, it lashes out."
Yeah... check out Belligerent Hatchling. That's an albino Medusa-wolf that spits bees. Why? Who knows.
Lorwyn art is usually awesome, but is often thematically incomprehensible. Primalcrux might be a case of the opposite problem: you know the thematic material (wood elemental!), you just can't understand any other aspect of the art.
If I hazard a guess, I would say it's some sort of antlered beast -- probably a moose deer et al -- the two 3-pronged clusters are legs (unusually widely placed), and there's also some random wood sticking out above the left(rel to beast, our right) leg.

The glowy bits are the eyes, with two four-pronged antlers above them. On the bottom are its two forelimbs, which are just random bundles of wood, some of which is jutting out, forming claws. The rest of its wooden body appears to be worm- or snakelike and is disappearing into the mist.
Thanks to everyone, yes okay I think the consensus that it isn't an elephant and those yellow things are eyes, and beyond that who knows. I really like the card but the art "isn't to my taste".

The closest you can get is definitely some sort of ungulate, either a primitive bull or a moose.
