Rise of the Eldrazi titans: Annihilator is a dumb mechanic and the near-ubiquity of these guys in casual EDH means I get to see that wonderful mechanic in action. A lot.
Swords to Plowshares: It's okay that this card made white better at removing target creatures than black because you're giving your opponent life! You're helping him win! Oh and Path to Exile was dumb design too. And that's why I extensively use these cards...
Cancel: I'm not one of those spoiled brat blue players that long for their old type 2s but this card makes me cringe hardcore
Leyline of Anticipation: Having more than one player with this card on the field is a headache.
Creatures with Hexproof or better combinations of "try dealing with me" abilities: You see since it's a creature and not a bunch of instants countering/killing your stuff it's ok that your options against it are limited.
Counterspell. It's clearly not the most broken card they've ever printed, but when you consider how long it remained in print, it's by far their biggest mistake ever. While Counterspell was legal, the only time a hard-grind control deck wasn't by far the best deck in all formats was when something was about to get banned.
In-context: Tarmogoyf. I admit it's a good card, but a singular card making blue splash green just for that? Broken.
Edit: you can't Bolt it to death if it's 2/3 with no Instants in a graveyard. That's a bit counter-intuitive.
That's just false. Red-deck-wins and white weenie were usually very competetive, if not outright winning the World Championship on any given year.
RDW and White Weenie were "competitive" in the sense that they could occasionally win a tournament in the event that all the Counterspell players basically forgot they existed. If the control players respected these decks at all they got completely crushed. When Counterspell was legal, it was so dominant that occasionally all the control players would start taking out all their anti-aggro cards to gain an edge in the Counterspell mirror. U/W would take out Wrath of God and StP, etc. When enough people did that, RDW could steal a tournament. After that everyone put their anti-aggro cards back in and RDW was completely dead.
I started playing when Type II was Urza/Masques. Counterspell was totally dominant and it just got worse and worse. If your opponent showed up with Replenish, or Accelerated Blue, or Psychatog, or Wake, or U/W Decree of Justice control, and you weren't playing Counterspell yourself, you could basically pack it in, you were screwed. There was years of MTG where after about round 3 of any tournament, the best opening hand you could hope for was 7 lands, because that was the best hand in the control mirror and that's all that would be left.
Miracles.dec really triggers me. Sensei's Divining Top being played by slow and/or unexperienced players is just so. damn. painful. Or "uhm, activate SDT, uhm, then fetch, activate SDT again, fetch again, another look in SDT, uhm Brainstorm, go, in your attack phase draw with SDT, Terminus, lol ggwp" Things like this are just dumb.

I really hate cards such as wellwisher, which give players absurd amounts of life. I can't stand having someone in my sight, thinking I am about to kill him, who then suddenly shoots out of range with life gaining cards. I only play multiplayer games, so this is quite annoying as it makes games last way too long. Other variants include Armadillo Cloak and especially Congregate.

No really, I used to both hate and LOVE Blinking Spirit back in the day. Back in those times, you could put combat damage on the stack, then return the Blinking Spirit safely to your hand but strill get to deal the damage! Incredibly obnoxious card. When I learned this it drove me nuts till I built a UW control deck with Blinking Spirits, Clones, and Doppelgangers. I would copy/clone Spirits for a beatdown plan, and if the opponent played something good, I'd return the clone to my hand with Spirit's ability, then copy his newly summoned Shivan Dragon, and then Swords the original. It also let me cast Wrath but keep all my creatures to redeploy.

If you loosen your definition of "out of the battlefield" slightly, then almost every single blue sweeper counts, too.

but, other than False Prophet, to deal with the likes of Ball Lightning or manlands, for example, all of those need Leyline of Anticipation or some other unplayable card to allow sorceries to be played as instants.

The Great Aurora also. I mean, yeah, it costs eight mana, but that's what you get to get rid of creatures in green. TGA and Worldpurge also hit things like Selenia, Dark Angel that can bounce.
Interestingly enough, there are a small number of "slow-blink" guys who survive. Ghost Dad for instance.

Card advantage is not the same thing as card draw. Something for 2B cannot be strictly worse than something for BBB or 3BB. If you're taking out Swords to Plowshares for Plummet, you're a fool. Stop doing these things!
I'm a standard player, and there have been a few cards I was glad to see rotate out. Particularly, Crackling Doom. A guy had a deck built around this that was impossible to beat. Down goes your best creature, 2 damage to your or your planeswalker, and it's an instant? There was nothing you can do about it. It's a sacrifice, so no protection from color, indestructible, or regen can save you. I wasn't pleased with his Mantis Rider either. Flying, vigilance, AND haste? Really?
Currently, I am waiting with bells on for Sylvan Advocate to rotate out. For all the reasons mentioned previously. I mean a 2/3 that costs 2 is still a really good card, but tack on vigilance and +2/+2? Gack!
I could also do without Reflector Mage. It's like I have to run blue counters if I don't want all my creatures bounced back to my hand. I have a little bit of an issue with Archangel Avacyn, but that one is not going away for a while. For now, I can live with holding on to an instant removal spell and just never attacking when I see five lands open.

I personally have found a newfound hatred of both Snapcaster Mage and Ghostly Prison because of my most recent PPTQ. (I played R/B Dash, we have no enchantment removal)
Snapcaster Mage: Had to play against this Match 3; He was playing Jeskai Harbringer; T4 Nahiri lead to dashing out a T4 Alesha's Vanguard and just flipping off Nahiri. He had 4 FLIPPING SNAPCASTERS IN HAND AND A HELIX, BOLT, AND PATH IN THE GRAVE. I still lost, was extremely salty. He never drew another that game.
Ghostly Prison: Ok, Match 6, no way into top 8... lets play thi- And t3 Ghostly prison. I stayed optimistic, and didn't scoop, I had 4 copies of impac-.... Thought-Knot Seer and Eldrazi Displacer... :| Really? And that happened in the second game also. FML. I went 2-4 good for first pptq.
