I think I've got another card I can add to the thread: Covenant of Minds.
Man, I hate this card. While I understand that Concentrate may be a little too powerful in certain environments, Covenant of Minds is just absolute garbage. First, it costs 5 mana. This is a reasonable price for a draw three spell, but it's also a sorcery. Jace's Ingenuity is an uncommon with the same exact casting cost, AND it's an instant. To make things worse, you also have to reveal the three cards you draw off of the Covenant. Now, a sorcery speed Jace's Ingenuity is already a great deal worse without even having to reveal the drawn cards, BUT IT'S EVEN WORSE THAN THAT because if you happen to reveal a crucial card to your opponent they can deny you the ability to draw it by having you take five random cards instead.
At the right casting cost punisher cards can be interesting, even if they aren't good. Overcosted punisher cards just suck.

I was convinced this card was a new card from Conspiracy 2, and was about to argue that it's clearly designed for casual multiplayer; but it's not. It's from alara and you're absolutely right! (Covenant of Minds)
Iona, Shield of Emeria. I dislike everything about this card. Oh, I just cheated it out onto the table on turn 2? Congratulations with having a deck full of worthless cards.
Or the extreme uber fun times that is an opponent going EOT Gifts Ungiven dumping Iona and Unburial Rites. Oh, I suddenly can't play 2/3 of my deck anymore? Out of nowhere? That's cool. I wasn't trying to, you know, play Magic or anything.
selvala, explorer returned. One of these in my playgroup that used to literally be based on untapping selvala. We would sit there for 20+ minutes waiting to see if they would get hurricane mana, if not the turn would pass and the guy who drew like 30 cards gets to win now

It used to be if you wanted to play next hate cards like this they were artifacts or enchantments. Running these had an opportunity cost of diluting your agression, which face the decks you were hosing a little time to deal with your hate pieces. Most of the best have cards can beat you up while they stymie your game plan.
All in all this is pushing creature strategies harder and harder. Every other strategy is getting hosed, and I think it's a crying shame.
Fetch lands, Mana Crypt, Wasteland, Sensei's Divining Top.
I go looking for ideas for a deck, and they're always listed. Really bugs me.
I actually like this about these cards. If/when I retire an EDH deck, those cards are ready to go into a new home easily. They're not just dead and need to be traded away.

And I hated the fact that the guy casting it is usually the type of guy to also play Hive Mind alongside it. Specifically because resolving Warp World for 20 minutes is not my idea of a good time.

Ugh. When I was in high school almost everyone played Affinity, and then Jitte came out, and almost everyone played Affinity + Jitte. I have equal disdain for both.

For me, Plummet. TBH, I don't see why green can't just drop the flying hate thing altogether. Now that we have fight, it's really not required for anything, and I don't even remember it being played much in Dragons of Tarkir limited. And what's hilarious is when these are off block theme too. (Leaf Arrow, Crash Landing)
The hell of it is, the only reason anyone ever played Hurricane is so they could play monogreen elfball. While Wellwisher is still in green's pie, the "damage to all players" aspect of Hurricane is decidedly not.
Equinox Vorthos concerns mostly. The big problem is, you have to be able to determine if a land will destroy a land you control. On top of that, it's confusing because of what "destroy" means. (Bolting or -2/-2ing an Assembly Worker isn't destroying a land. Shattering or Blading one is. Edict effects like Cataclysm doesn't count; nor does exiling or bouncing one. And it doesn't effect abilities.) Fortunately, it's...kind of a bad card. Not Wood Elemental bad, but definitely "You won't see this at a tournament any time soon." bad.
Goblin Game I really wouldn't mind this card in silver border, but in black border, it's an abomination.
Cards that dilute the sanctity of exile à la Pull from Eternity are another personal trigger for me.

Card advantage is not the same thing as card draw. Something for 2B cannot be strictly worse than something for BBB or 3BB. If you're taking out Swords to Plowshares for Plummet, you're a fool. Stop doing these things!
Sol Ring: Sol Ring in fine in EDH and Vintage because its restricted. However, if you are the one who ventures into in new play groups often and stumbles upon "That Guy" with four in his deck then it becomes a headache. Most casual play groups are no hold barred and a lot of times people are too polite to stand up to one or two players being jerks. Again, not the card I dislike just the people who justify using its power under the vague definition of what a "casual format" is and isn't.
Geralf' Messenger: I get a lot of flack from my friends for this one. I hate to -1 myself and coupled with being slapped in the face with 2 life made me really dislike this zombie.
Messenger off Cavern of Souls made really salty back in the day.
Delver of Secrets: Again, not the card I dislike but the jerks that seem to gravitate towards it. It seems every Delver player I've encountered is some obnoxious jerk that seems to think more Islands in your deck equals more skill. Anything that beats Delver is obviously overpowered and Treasure Cruise was falsely banned.
The other one I don't like is extort. Mostly I don't like the idea of white getting Drain Life-type effects. Or really any color but black getting those effects. And RW multicolor (not hybrid!).

1. Tarmogoyf: This card bugs me, it power and toughness fluctuation is annoying to keep track of, doubly so because the +1 toughness aspect, I wish so badly this could be reprinted as simply */* power
2. Fetchlands, I hate excessive searching and shuffling, slows the game right now. This is fine for spells, but such an annoying effect on lands. Wish fetchlands were never printed.
3. Excessive graveyard interaction. Flashback is fine, you only need to concern yourself with a few cards. Dredge type mechanics that create huge graveyards are a pain to keep track of what the opponent is doing.
4. Cards that interact with cards already in the exile zone. Ulamog's DespoilerCryptic Cruiser. I already need to keep track of the opponent's graveyard, now I need to keep track of Exile too? I can't wait until they create the Super Duper Exile Zone.
5. Cavern of Souls nearly conditionless protection from counterspells, not fun.
Any 2 easy-to-use cards that ruin an entire strategy unrecoverably.
