I am in the process of double sleeving some cards, all inside KMC Perfect Sizes + KMC Hyper Mattes.
I'm putting the cards into the Perfect Sizes upside-down, so that the opening on the Perfect Size is not exposed when I put the card right-side-up into the outer sleeve.
Here's how I do it: I put the card into the outer sleeve just a little, then I turn it upside-down and gently press it against my playmat so that it slides the card into the sleeve. If I don't do this, then the inner sleeve slides up the card as I'm sliding it into the outer sleeve, and the inner sleeve will stick out the end of the outer sleeve. That's obviously no good. But when I do my upside-down press against the table method of putting the inner-sleeved card into the outer sleeve, it only goes as far as until it's completely flush with the opening of the outer sleeve. Also no good.
So what do I do? I turn it back right-side-up and I grasp it between both hands so that my thumbs meet in the middle of the front of the card, and my middle/ring/pinky fingers rest in the middle of the back of the card, with my index fingers resting on the top over the opening. I push down with my index fingers, which slides the inner-sleeved card the rest of the way down into the outer sleeve.
This method has been working perfectly for me.
Until now.
I've got one card that just *will not cooperate*. I've tried putting this thing into the sleeve 100 times, I've changed the inner sleeve, I've changed the outer sleeve, I've tried alternate methods that I don't try with any of my other cards, and NO MATTER WHAT I DO, when I try to press it down further than flush-level, the CARD slides down, while INNER SLEEVE moves up and sticks out of the opening of the outer sleeve. And I'm not kidding, I'm at the point of just putting the damn card in the shredder (it's a Fleecemane Lion for what it's worth). I've used the same sleeves as I *can't* get to work with this card on other cards, and it works just fine. For some reason, THIS card, will NOT go into a double-sleeve, the card slides out of the inner-sleeve and the inner-sleeve sticks up out of the opening of the outer sleeve.
Is there any other way to do this? Has anybody ever had a situation where it just seemed like one specific card would not freaking cooperate, as opposed to it being the sleeves themselves? I've had this before where the sleeves were the problem and as soon as I put the card into a new inner (or outer) sleeve (or both), I was able to sleeve it with no issue, but this is the first time I've ever seen where the card itself seems to be the problem.
Try putting an unsleeved card in behind it at the same time.
EDIT: That might not be a clear explanation. What I'm suggesting is you have 3 things: the Hyper Matte sleeve, the card inside the Perfect Fit, and some random unsleeved card/token.
Put the edge of the Perfect Fit card inside the Hyper Matte. Then, put the edge of the random unsleeved card into the Hyper Matte, behind the Perfect Fit card. Push both the Perfect Fit card and unsleeved card down together, then pull out the unsleeved card. That's always worked for me.

Thank you for this suggestion! It worked! It took me a couple attempts, because I had trouble pulling the unsleeved card out without also screwing up the sleeved card, but I figured it out. This is definitely a great thing to know for when I inevitably have this difficulty again.
Er, I'm not really sure what you're describing. When I double sleeve (both KMCs just like you), I do the Perfect Fit sleeve first, without touching the Hyper Matte. Then, when it's sleeved (upside down) in the Perfect Fit, I put it into the Hyper Matte. Just make sure the whole circumference of the Perfect Fit is beyond the opening of the Hyper Matte and then just gently push down.
If the card is falling out of the Perfect Fits then it's obviously not snug enough and there's nothing I can suggest except maybe it's slightly miscut and a tiny bit too small.
Yep, I never had this problem. Slide into perfect fits - push perfect fit into hyper matte and that's it. I push my thumb inside the hyper matte to get it (the perfect fit) as deep inside as it will go. Never fails.
I have a ridiculous amount of trouble, I can't figure out why. If I don't use the table top surface to help push the card most of the way into the hyper matte, then it's almost impossible for me. I don't get it. That said, I have various tricks that seem to work for me.
I've ran into this before. Sometimes sleeves are very slightly larger or smaller from general variance in manufacturing. I slightly bend the card so I can fit my finger between the inner and outer sleeves and push from there. I'll try using the 2nd card method next time it happens to me.
This comes up with old worn cards, alpha cards and certain foils also. The Perfect Fit sleeve just slips partially off the card when inserting it into the outer sleeve. As noted using an additional card to ease the card in helps. This issue arises most often when using new outer sleeves. When using older ones, which are less rigid, I rarely have issues even with cards that seem impossible to get into a new outer sleeve.
One thing to note is that problems with card size can be a mark of fake or sheet cut cards, but most likely the issue is with sleeves. One version of Portugese fakes during Onslaught era was about 85% of the size of regular cards. Those were impossible to double sleeve. (Local store has few for teaching how to spot fakes.)
It can also have to do with the humidity/temperature at which you've been storing cards; if a card has some play-wear at the edges and your room is cool and humid, the edge-worn card can deform slightly which makes sleeves not behave well.

I have found that if the card doesn't slide in all the way with the perfect fit sleeve on, and the sleeve lifts up, I just pull the card/perfect fit out turn the card/perfect fit upside down and then put it back into the outer sleeve and push it all the way in (card is now upside down). Let it sit overnight, or a short period of time to stretch the outer sleeve a bit and then the next day I pull the card/perfect fit out, turn it right side up and slide it back into the outer sleeve no problem
This is with KMC perfect fits and KMC Hyper Mattes.
Found this thread because I also run into this issue sometimes. I generally use KMC perfect fits and UltraPro Eclipse outers. I have a couple sets of arty sleeves that show a commander. The latter type more often causes the issue of the outer sleeve pushing the inner sleeve off a bit, for the couple millimeters that the inverted table-press method doesn't cover. I hadn't yet thought of using another card to reinforce the last push, so thanks for the idea!
And of course at this point, I think I might reverse the way the cards are sleeved, with the opening of the inner sleeve at the top and the outer one at the bottom. This way, should anything fall into the deck box while it is sitting open, it won't fall down into the outer sleeve. I guess I need to switch to Bordifies inners anyway, to make the few white-border cards I have blend in better.
I'm using Dragon Shield inners and outers, and generally run into this issue with brand new outer sleeves. Once they have had cards in them for a bit, they tend to be a little less tight and the inners give me less trouble.
