I wondered if anyone has ever pulled two of the same card out of a booster pack? I mean two normal copies, not one normal and a FOIL copy as well.
It occurred to me that a guy I played at the pre-release had a R/B mostly burn deck with two Bolt of Keranos in it, and there was only one pack of Born of the Gods in each box. So I was curious what the odds were that two of the same card came out of the same pack.
It is one of the more common examples of an error pack. I played a 3xTheros draft just before the BotG release, where 2/3 of the packs had duplicate uncommons, so it's still a possibility.
While players should technically call a judge when they open one, most people don't pay that close attention to commons until after they've already sorted them.
During Lorwyn, Eventide, or Shadowmoor timeframe I had done a draft at the LGS where half the box had 4-5 copies of the same blue merfolk in each pack. I can't remember the exact card but everybody wondered how the hell that happened.

In "german" PreReleases and Boosters such errors happen often, much much more often than in english product.
Had stuff like "flipped" cards in Boosters, flipped to the side, flipped entirely with the back and any form of errors imaginable.
Boosters with "all rares" or "all commons" , boosters with priting errors and the list goes on.
So yes, such stuff happens.
If it happens, call a judge immidiatly ; most often they will notice the error and just let you play with it.
In some cases the error is bigger and kinda problematic (like a Booster with all rares) , so you keep the booster and get a new one (if possible).
Especially on Grand Prix and the like, ALLWAYS call a judge over if you discover any error in your product, in now way keep playing with it on your own, it will just give you problems, especially if they actual "deck check" you.

I think I had another pack with 2 foil cards that were the same. I think it was a Theros pack with 2 foil divinations.
Packs are not SUPPOSED to have multiples of the same card unless it's in the foil slot. But like crimping, and printing errors, it does happen every so often. I would have called judge just to be on the safe side, as it would be very easy to cheat in an extra common at a prerelease like this. Thought Lightning Strike would have been more effective if was trying to gain an advantage from cheating, and less noticeable IMO.

I opened a Theros pack with 2 omenspeaker, yesterday. I don't draft or played sealed much so I never gave much thought to it. There are probably other times I haven't noticed. If it is a printing error, I wonder if the new electronic scanning of cards with the new frame update will eliminate this?

The whole pack was sort of off-center as far the cut goes. I guess they just had a bad miscut sheet and had to shuffle some cards around. Most likely reason to get 2 of the same card in a pack is miscut sheets and cards getting shuffled. Anyways, if you get abnormal product in a draft or sealed pool you are supposed to call a judge as soon as you notice.
Thanks for letting me know. Yeah, my opponent just had a wicked burn deck that kept my creatures in check too easily, splashed black for kill spells. I just had wondered.
FYI: This will probably happen a lot less or not at all once the M15 border is live since one of the reasons for the change was a change in the card number text to make it readable by automated scanner sorters.
I personally have never had it even when I had packs with more than 1 rare. I haven't purchased too many packs in the past 10 years short of Limited though.
