In the game of Magic so far (including Unglued and Unhinged), there are 719 cards that can generate creature tokens. (This only counts playable cards, not special cards such as those from Archenemy. It also counts double-faced cards from Innistrad as two separate cards.) Between them, they can generate exactly 271 unique tokens (not counting unspecified tokens that enter the battlefield as copies of other creatures, etc.). Here is a list of each unique creature token so far, up to and including Fate Reforged.
The following list is arranged by creature (token) name, then by color, then by P/T, then by ability. Anyone up for creating customized tokens for the remainder?

It seems you have wrong art on blue-white Bird and Goldmeadow Harrier, at least...
I used the unofficial blue-white bird since it featured the hybrid background (you can tell what colors it is much easier than the gold token background) and it was simply a much better picture.
I think you're right about the Goldmeadow Harrier, though. Do you have a link to the post with the correct art?
QRedux: I'm not sure if you realize that there is already a thread full of tokens. Although, you are free to make more if you feel like it. That Orb token is sweet. Also, its better to have artist credits whenever possible.

Just one question - since this token is named "Llanowar Elves", can you use it it as a regular Llanowar Elves?
Not really, since the real Llanowar Elves has a casting cost of G, whereas a token doesn't have a casting cost. So a card like Droning Bureaucrats can affect one but not the other if both are in play.

The token from Wall of Kelp is a Kelp Wall.
If it is custom, I was wondering if anyone could make one without the angel looking like she needs to eat a sandwich or two...
