The card that has the most sentimental value is my Divine vs Demonic Akroma, it was my favorite card out of that duel deck which was my first magic product
My very first card, is probably tied for most sentimental, an 8th edition Shatter--this is tied with a foil Kiki-Jiki, Mirror Breaker, one of the last cards I got before taking a break when Kamigawa ended. I came back just before New Phyrexia came out and at the NPH prerelease I pulled my first planeswalker: Koth of the Hammer and he is right up there. PWs are my favorite card type...
The most expensive card I own is probably the foil Sword of Light and Shadow that I keep tucked in the absurd page in my collection binder with my non foil light and shadow and non foil fire and ice and Ach! Hans, Run!
Most expensive and nostalgic is probably my Grim Tutor-because I found it in my garage in a small pile of cards that my older brother used to play. Or the Chaos Orb I won at an EDH Tournament.

Most Sentiment: my 7e collection given to me by my best friend who was moving to a different town about the time 7e was released.
Most sentimental is probably going to be a card I just got. I've been trekking through China, and came across an MtG dealer. I picked up a few singles from him, as well as five Chinese Innistrad boosters as a souvenir. I was happy simply having Chinese cards, but also really wanted (who wouldn't) to pull something memorable. I ended up adding a Chinese foil Geist of Saint Traft to my collection

Most expensive: Revised Underground Sea's
Most sentimental: My altered art Scrubland / Badlands/ Plateau by Sandreline and my Shards of Alara Elspeth foil (all in my Kaalia EDH deck).
If I could find it, I'd say my Tarmogoyf. I got a great deal for it on eBay, put it in my cube and even used it a couple times. Last week, when I was going through my cube to reconstruct my lost card list, it wasn't there and I have no clue what happened to it.
As for cards I can lay hands on, I'd have to say it's my Gaea's Cradle, also a resident of my cube. I got it (along with a ton of other cards) from a cousin who had stopped playing years ago. If only I hadn't traded the Force of Will I got from the same cousin. *sigh*
In sentimental terms, it's my foil Seventh Edition Thorn Elemental hands down. I got it out of the starter deck that was my first Magic purchase ever and it was the all-star of my first real deck. Sure it doesn't have any protection from removal, but who cares!? If it attacks and lives, your opponent will take 7 damage, blockers or no.
Most valuable card? Signed playset of Onslaught Polluted Delta. I was late to the tournament scene, and unfortunately only got into Legacy as prices were starting to shoot through the roof and beyond my reach.
Most sentimental? My 6th edition Doomsday. This was the rare I opened in my very first pack of MTG. I've held onto it for 13 years, no reason I won't be holding on to it for 13 more.

Hmmm, my top card at the moment would be my unlimited mox sapphire, followed by an unlimited mox jet, followed by library of alexandria (but I'll be acquiring an unlimited lotus, the other 3 moxes, and an ancestral recall at unbeatable prices from someone I know within a few months when I have the cash to spare), followed by revised underground seas, other revised dual lands, then probably tarmogoyf, foil liliana of the veil, foil vindicate, foil snapcaster mage, then it's a toss up between several cards in the $50-70 range including force of wills, ali from cairo, other revised dual lands,... and just a few days ago I got myself a nm foil Japanese phyrexian obliterator which is a nice addition my binder. I'd definitely have several more big cards if I held onto them years ago and didn't quit playing back in '98 like the douche that I was, but eventually I'd get the remaining power cards along with other staples like moat, mishra's workshop, bazaar of baghdad, and a few others back in my collection. All in due time... *sigh*
Anyhow, the best I can come up with myself is a game in the top 8 of a PTQ back during Urza block in which we were starting game 3 with time already expired, so the tiebreaker rule was that whoever had more life after 3 turns would win. And I lost to... healing salve.
Mine would be my Foil Rishadan Port in my Ulamog EDH deck. A friend of mine gave it to me for a present a while ago. Its in near mint condition as well.
Edit: Most sentimental: My set of 85 (and counting) foil Pillarfield Ox, one of which is signed by a buddy of mine to commemorate my mass polymorphing into 3 Pillarfield Oxen for lulz against him at an fnm and beating him with the stampede of oxen. He was playing valakut. Lulz did ensue.
