I pose this to the community at large...what cards do you consider to be Nightmare Fuel?
For me, three stand out. Mutilate's original art was greatly disturbing, seeing people being twisted and distorted, the agony on their faces, very real. Secondly, there's Tormentor Exarch. Unlike the rest of the Exarchs, his lower half is still quite human...there is a human being under there somewhere. And finally, Forced Worship just...is very, very unsettling for me. Brrr...

The Jin-Giataxian surgeon dude, whatever..omg that card is pretty awful. The one where the subject's head is peeled open and he's about to dive in.. edit: Sensor Splicer I guess it's not that bad..it creeps me out though.

Takklemaggot is also pretty foul. (if you look closely, you can see the skull reflected off its compound eye. Notice it's not smiling. :uhh:)

I have heard vague rumors of a moustache-dispensing vending machine in a distant laundromat, across the street from a tattoo parlor. However, this information is shaky, and time is of the essence.
You know these... things? Twisted humans that crawl on their feets and hands, updomen on the upside, head twisted 180 degrees? First time I admit this, but I have ****ing nightmares everytime I see something like that. They are not looking like that Thrull, to be honest, because they are twisted another way. But Basal Thrull reminds me of them. *brrr*
I think "Unborn" had a scene with such a horror, in the home for the elderly. It freaks me out. And then there is this fake-gif of such a thing crossing a road in the woods... To be honest, I just googled to look up what these things are called. At least I tried. I could not do it. I could NOT click a few links and watch some pictures of a fictional creature. Cold sweat included. And I can barely look at Basall Thrull. But the worst thing is... I DONT KNOW WHY!
But my number 1 freakiest are tied between Phyrexian Unlife and Abomination. The first is deeply disturbing, and the second looks like a monster made of bubble gum.
Gaze of Pain from Ice Age is one that immediately came to mind.

Buried Alive is probably the most horrifying to me, flavor wise. It's pretty much the worst thing in the world to happen to someone, and the artist's perspective is brilliant yet terrifying.
EDIT: Lolz, I never saw the artwork for Worms of the Earth before, and while the bottom half is pretty creepy, the other half is pretty hilarious for a perverted mind. Love how the dude's bending over, and the worm on the left...yeah..
This isn't terrifying or anything, but every time I look closely at the art for Sensation Gorger, the thorns going through his fingers and twigs holding his eyes open make feel all sorts of uncomfortable.
Not art related, but MTG nightmare fuel none the less..
This past year at Anime Central, one of the guys in the Magic room mentioned playing Magic in a hot tub on occasion.
He pulled out a large 1000 card box, which turned out to be his Cube.
In it, were Power, Duals, and lots of money. All triple sleeved.
I asked about the hot tub comment, and he said to test the waterproofing of the triple sleeve setup, he had sleeved some Duals, tossed them in his bath tub and filled the tub. Then drained it entirely before pulling the cards out of the tub.
The first sleeve was the only thing wet. Didn't even make it to the second, so the cards were perfectly fine.
Not sure on why anyone would wanna even think about doing that, but I don't think I've ever cringed quite that hard at anything Magic related before. D:
Stated in Elesh Norn, Grand Cenobite's flavor text, the Gitixians speak of "other worlds" that would can very-well be Phyrexianized.
Here's a little bit of scare: Imagine Lorwyn, a fairy-tale styled plane, phyrexianized. That plane, mixed with the science fiction horror elements of Phyrexia. Two things that would never come together, but would.
Same applies for Kamigawa, the Shards of Alara (even Grixis), and heck, maybe even the Eldrazi*.
* I'm not sure if the Eldrazi are able to be Phyrexianized to begin with, but if it is the case, that's just making one dangerous thing even more dangerous.

Also, I can't remember the name, but it was an older (Odyssey I think) Blue card of something EATING another guys Brains... Like a leech, eulch!
Inquisitor Exarch also looks just...creepy, he's too thin to be a normal human, and the whole Cenobyte look is just, scary, ya know?
