I know you get 11 commons, 3 uncommons, and a rare/mythic, but how do foils appear? Do they come randomly as a 16th card? I'm asking this because someone said that his friend got a Karn and a Foil Sword in the same pack.
im guessing foil commons are more common than foil rares or something though right?? or am i just real unlucky?
Well, there are far more commons and uncommons in a set than there are rares. So you're more likely to get a foil one of those just because of probability.
In some older sets, the rarity of the foil replaces a card in the pack (ie if you get a foil rare, you don't also get a regular one).
The foil card probabilities:
11/16 common
3/16 uncommon
7/128 rare
1/128 mythic
1/16 land
Chance of opening a pack with a foil in it:
15/63 chance (roughly 24% or 1 in 4 packs)
So to answer your question, the foil card replaces a common. Even if it's a foil land (this is why you have to register foil lands at tournament level sealed events, because the foil land has replaced a common).
So, chance of getting a foil pack: 24%
Chance of opening a pack with a mythic in it: 13%
Chance of a foil card being a mythic rare: 0.7%
Chance of opening a pack that has a mythic and a foil mythic in it: 0.0232% or roughly 1 in 4,300 packs.
Foils Replace a common card slot, so it is possible that your friend pulled 2 mythic, 1 foil and 1 non foil (I did that with Worldwake, did a foil jace and a normal jace in the same pack, my luck has gone underwater since then = =)
I remember they lowered down the ratio of the foil card appearance, I remember back in when foils first came out, it was around 1:75 cards (so thats around 1 in 5 booster packs) but now that % has been lowered..
I just found a Zendikar booster pack (I have no idea why)
Ah here we go, if you look at the back, it will say: odds of Premium cards: 1:67. So you will see a foil card in every 4 or 5 packs nowadays. I guess thats why new released foil cards seems to be cheaper than the older cards

What's the probability of opening an Innistrad booster that has 3 rares in it?
Assuming the double-faced card sheet is the same distrubution as a foil sheet and the math above is accurate for INN -
.875*15/63*7/128*7/128=0.0006 or about one in 1600
