I'm guessing (so don't kill me) is that it's similar to the whole rip-a-topdeck-card styled thing. Granted, there's not a full windmill in that, but ykno. I've seen a lot of good "JUST RIP IT!" scenes, the best being at the Gunslinger tables, when a guy ripped Siege-Gang for enough Gobbos to feed to Clickslither for the win on the attack. (They were using the Elves vs. Goblins decks.)
That aside, with Lorwyn, it's easier to pull them off, since you may end up relying on Clash to win. Rip the revealed card, see what you can do. Think Lash Out.

It can be used in different contexts. If used while playing, it usually refers to winning the game off of a super top deck (but that usually called a Mise...see the non-arena promo for more info) or them walking into your perfectly set up trap. In draft, it would be the card that once you see it you automatically draft it...like seeing Shriekmaw.
You could expand it to "any dramatic play/pick in a magic game". But that is probably overgeneralizing.

Basically what Memnarch said, I'd suggest taking the winning topdeck up over your head and slam it down on the table. Also, say something really cool while you do it, like "This is Sparta!" Or just say something nonsensical like "Jumanji!!!!!!"

I always thought it was used in the context of a draft (too), like if you open a foil Garruk in a Lorwyn booster draft first pack first pick, you slam it in your pile fast no matter what.

My personal example of a windmill slam: Tuesday night I was playing in a sealed league. Someone dropped turn three E.T. My turn, I rip a Lignify, and slammed that baby onto ET faster than a Raging Goblin. They weren't too happy, but hey, it saved my can. And made my Battlewand Oak stronger.
Windmill slam?? I was thinking more on the lines of Zangief?? Anyways that is what I picture a windmill slam and I tend to do that alot to complete strangers and stray dogs.....:D
tl;dr: Nassif pulled a windmill slam when he flipped the rite of flame at the last copy of Chapin's Ignite memories during game 4 of the World semis. copy that and you'll be good.
Some may think Craig Jones' Lightning Helix was a windmill slam, but after watching the video, it was merely a flip. (Twas certainly worthy of a slam, though!)
