Planeswalker - Ashia
+1: Exile target card in a graveyard. If a creature card is exiled this way, create a 1/1 white Spirit creature token with Flying.
-2: Create three 1/1 white Spirit creature tokens with Flying.
-5: Destroy all non-Spirit creatures. You gain life equal to the number of creatures destroyed this way.
[2]
EDIT: I also noticed that Severance's post got skipped, so here's his elemental walker (Flamekin are Elementals, so I went with that):
Embry of Flame's Path 4R
Planeswalker - Embry
+1: Add RR to your mana pool.
-1: Put a creature card from your hand onto the battlefield. It gains haste. Sacrifice it at the beginning of the next end step.
-7: You get an emblem with "Whenever a creature you own dies, exile it instead, then return it to your hand at the beginning of your next upkeep."
[4]
Victory 3BWR
Planeswalker - Koth
+1: Target land becomes a swamp until your next turn. It loses all other types.

- Untap all creatures you control.
- Untap all artifacts you control."
4
Next: What would have happened if Ajani had gotten to the maelstrom first?
Ajani, Maelstrom Reborn 4WBGR
Planeswalker-Ajani
+1 Cascade the next spell you cast this turn
-2 Return target permanent from the Graveyard to the Battlefield
-10 You get an emblem with "at the beginning of your upkeep look at the top 5 cards of your library you may put any number of permanents from among them onto the battlefield"
(5)
next: A planeswalker that hangs out with living swords
Errenet, SwordancerWUB
Planeswalker - Errenet
+1 Create a 1/1 colorless Sword Spirit artifact creature token with flying.
-2 Reveal the top five cards of your library. ~ deals 1 damage to target creature for each artifact card revealed this way. Put one of them into your hand.
-6 Exile target permanent. Artifact creatures you control gets +X/+X until the end of turn, where X is a converted mana cost of the exiled permanent.

Spoiler for second season/book of Game of Thrones
Joffrey Baratheon2RB
Planeswalker - Joffrey
+1 Draw three cards. You get an emblem with "Spells you cast cost 1 more to cast."
-3 Name a creature card, then exile all creatures with the same name.
-9 If there are three or more opponents in play, target player loses the game.
Joffrey Baratheon can be your commander.
3
First ability represents his bad reign, second one - cruelty and executions, and the third one - winning in War of the Five Kings
next: choose a character that already has a card, create a three-color or four-color version, and support it with flavor. For example, Ajani venges for Elspeth and goes mad, so he is WBGR
Sarkhan, Successor of Bolas2UBRG
Planeswalker - Sarkhan
+1: Destroy target permanent that isn't a Dragon. You lose life equal to that permanent's converted mana cost. If that permanent is a land, you lose 3 life instead.
+1: Put two +1/+1 counters on up to one target creature. That creature gains flying and trample and becomes a Dragon. (This effect doesn't end at end of turn.)
+1: Dragon spells you cast cost 4 less to cast until end of turn.
-7: Create a 10/10 red Dragon Avatar creature token with flying. Draw ten cards.
{[5]}
Not too much of a stretch there; Sarkhan's already BEEN all of those colors at least once.
Next: A "team up" planeswalker with two planeswalker types, where both of the planeswalkers involved are dead at the present in the storyline.
The Nine Titans 1WUBRG
Planeswalker - Urza Freyalise
+2: Draw two cards.
-3: Destroy target permanent.
-X: Destroy each other nonland permanent with converted mana cost X or less.
The Nine Titans can be your commander.
{[5]}
Yeah this is the only proper old school Planeswalker team up I could think of where everyone was dead. I only put Urza and Freyalise down for the planeswalker types since they're the two most iconic names in the titans and one of them is already a walker. Also commander because we all know a true superfriends EDH would have a planeswalker leading them.
Fblthp, Cowardly Homunculus1WU
Planeswalker - Fblthp
+1: Create a 1/1 blue Homunculus creature token with skulk.
0: Put Fblthp on top of your library.
-4: You get an emblem with "Sacrifice a Homunculus: Prevent all damage a source of your choice would deal this turn."
[2]
Next: A Planeswalker with a non-loyalty activated ability.
Kaya the Ectomancer2WB

[-2]: Create a 1/1 white Spirit token with flying. Target opponent sacrifices a creature.
[-8]: Destroy all creatures you don't control. Return each creature in your graveyard to the battlefield.

...You seem to be thinking of a different card, neither Narset card uses keywords that are set-specific. Rebound also shows up in Rise of the Eldrazi.
Narset, Light of Aretopolis3WU
Planeswalker - Narset
+1: Until the beginning of your next turn, whenever you cast a noncreature spell, prevent the next 1 damage that would be dealt by a source of your choice this turn.
-2: Put target creature on top of its owner's library. You gain life equal to that creature's power.
-7: You get an emblem with "You may pay 1 rather than pay the activation costs of abilities you activate."
{[5]}
Next: That was Narset-on-Kephalai. Do another existing planeswalker visiting a plane that's never had a full set/block dedicated to it. (The list of all known planes)
Shandalar, maybe? In lore, it's been a place for spars between 'Walkers. I... don't know of any other Planes that aren't covered by a set. Are there any 'Walkers who's home planes haven't had a set for them that have enough lore to base a card on?

There are literal dozens of them in the lore, many of which showed up in Planechase and Planechase 2012, sets which a lot of people on this forum in particular like, so... maybe this challenge isn't for you. Maybe you don't have to do every challenge in every game. Maybe you could post elsewhere while someone else takes this one.

[4]
You next challenge is: A transform walker that changes colors.
Cathar Kord of the Goldnight1WR
Planeswalker - Arlinn
+1: ~ deals 1 damage to target creature. That creature can't block this turn.
+0: Creatures you control gain vigilance until the next endstep. Transform ~.
-9: You get an emblem with, "If a source you control would deal damage it deals double that amount of damage instead." and, "If a source an opponent controls would deal damage to you it deals half that amount rounded down instead."
3
// Arlinn of the Wildnight (R/G)
Planeswalker - Arlinn
+1: ~ becomes a 4/4 Werewolf creature with trample and indestructible until the next endstep.
-1: Target creature you control gains trample and hexproof until your next turn.
-4: Deal 4 damage to each creature. Transform ~.
Next: They chose to lock Sorin up rather than kill him. Why?
Sorin, Rebuilder of Innistrad2WB
Planeswalker -- Sorin
[+1]: Until your next turn, whenever a creature attacks, you may gain 1 life.
[-3]: Return target nonland permanent card with converted mana cost 3 or less from a graveyard to the battlefield under your control.
[-8]: Exile all nonland permanents you don't control and all cards from each opponent's hand and graveyard.
[4]
Silumgar, Fanged Sovereign3UB
Planeswalker — Silumgar (M)
Whenever you sacrifice a creature, put a loyalty counter on Silumgar, Fanged Sovereign.
[0]: Sacrifice a creature. Draw a card. You lose 1 life.
[-2]: Target creature gets -3/-3 until end of turn.
[-8]: You get an emblem with "At the beginning of your end step, if you exiled 5 or more cards from your graveyard this turn, gain control of target permanent."
[5]
Narset, Contemplative Rebel2WRU
Planeswalker -- Narset
[+1]: Create two 1/1 white Monk creature tokens with prowess.
[-3]: Reveal cards from the top of your library until you reveal a noncreature, nonland card. This turn, you may cast that card without paying its mana cost. Put the rest on the bottom of your library in a random order.
[-8]: Narset, Contemplative Rebel deals 8 damage to each creature you don't control. You get an emblem with "Whenever a creature enters the battlefield under an opponent's control, this emblem deals 8 damage to that creature."
[5]
Next: A GW planeswalker based on an existing Magic character other than Ajani.
Nissa, Nature's Mystic 3GW

[+0] draw a card. you may play an additional land this turn

[5]
next: A 3 color planeswalker based of the original Elder Dragon's besides bolas
Palladia-Mors, Planeswalker
Planeswalker - Palladia
[+2]Until the end of the turn target creature loses all abilities and become a red Dragon with Flying and 1/1.
[-3]Destroy up to two target creatures, and if you do, create a Red and White Orc creature token with 3/1 for each creature destroyed.
[-12]Exile all other permanents, and if you do, create a Green Zombie creature token with 1/1 for each permanent exiled.
[6]
I tried to reference Palladia's lore as much as I could. The +2 is a reference to Vaevictus being turned into a dragon whelp and Palladia reversing it, and the -3 and ult are references to the fact that Vaevictus and Palladia had an army of zombies and orcs.
next: Create a planewalker which is essentially a planeswalker whose body is missing for whatever reason, and have their soul bound/trapped inside of a magical construct from which they are using their powers. Feel free to either use an existing planeswalker to have trapped inside, or create an entirely new one.
