I'd made Outwit a while back, called it Wardhelm. Also made Deathly Bite with green mana instead of black. Grounded is also one of the most common forms of flying hate I put in my custom sets, though I named it Overburden. This is the first time I've had any cards to post here at all, I'm surprised it was three at once.
I made Abundant Growth... the only mechanical difference is that mine cost (2/G) because it was part of a twobrid set. (Well, I made the design, but then I cut it in favor of other stuff. I don't remember what the name was.)
I posted a few cards similar to Dangerous Wager a while back, glad to see that such a thing became a reality (and better than mine, which was a sorcery).
Unhallowed Pact, as a black-shifted False Demise, is definitely something I've put forward as something I'd like to see.
Terrifying Presence showed up in my Kitsune thread a couple of weeks ago, though I believe credit goes to Scuirimancer if I'm remembering correctly.
The "biggest power matters" duo of Triumph enchantments is a mechanic I've dabbled with (named "Dominance") on some Muragandan dinosaur designs. It feels out of place to me on Innistrad, but whatever.
No cards stolen from me, but it's neat that Wizards took 2 of my keyword's names in just the Innistrad block. Sadly neither of these were stolen either, but I think the coincidence is cool enough.
Undying (Exile ~ from your graveyard: Place a token on the battlefield that is a copy of this card that is a spirit in addition to its other types)
Soulbond (As ~ creature enters the battlefield it is bonded with target creature. Whenever either creature is destroyed, destroy the other)
Created a card called "Quilled Mastiff" that got +2/+2 if it was damaged. Wizards made it better calling it "Wild Defiance". Even the creature depicted looks like a Quilled Mastiff.
Recently I've been adding Ninjas to my custom Cube, and added this little gem:
Minamo Deceiver2UU
Creature - Shapeshifter Ninja {R}
Ninjutsu 1U
You may have Minamo Deceiver enter the battlefield as a copy of any creature on the battlefield.

I made it about 5 days ago. The most recent spoiler from Planechase is Sakashima's Student. I guess its a pretty obvious thing to do with a Ninja, but come on!
In reference to "Destroy target creature." That will also put a rest to the discussion here as to what the correct cost is (at least currently it is 1BB).
Highly doubt I am the only person that ever suggested this, but I can be the first person to report having done so
Not a specific card, but in my first custom Core Set I started almost a year ago at least, I reprinted some Legends rather than making new ones like they are in M13. I had created some uncommon creatures to serve as their "second in command", which had abilities that helped make the Legends better. Wizards did just that with their new Legendary cycle.
It's not really a card that they've stolen from me, but I've had in my dropbox 3 files codenamed Blood, Sweat, and Tears for awhile, as my next (or possibly the one after) custom block. WotC just posted today that these are the codenames for the 2015 block.
I used to have a card called Hunter´s Song in an old, scrapped custom set. It gained counters everytime your creatures dealt damage to opponent creatures, and these counters could be spent for mana of any color
Naturally i see Druid´s Repository and i jump of joy because i needed another reason to go through my abnormal Eldrazi Spawn fanaticism
I invented living weapon before equipments came out. My version was called Centipede. The pieces were artifacts that had a cost and tapped to attach to target creature, including the 0/1 centipede token they created.
It's funny how the first Mirrodin block introduced Equipment that made me say. Oh I can build a centipede deck I just need to throw in a couple of small creatures. And then the next Mirrodin block introduced living weapon which actually allowed me to make the deck in purity.
Just to be clear there is no possible way Wizards got the idea from me.
I was looking through old pages of the the winner is judge game and saw this page where the challenge was to create cards with new subtypes and one entry had a card with a curse subtype and another entry had an ability called "soulbound."
Searing Profanity3BR
Enchantment - Curse {U} 1BR, Sacrifice a Curse: Searing Profanity deals damage to each opponent equal to the amount of Curse cards in your graveyard.
SkysoulUW
Enchantment - Bird Soul (C)
When ~ enters the battlefield, target creature gains flying until end of turn.
Soulbound - Creatures you control have flying if there are four or more Bird cards in your graveyard.
Lifesoul1WG
Enchantment - Druid Soul (U)
When ~ enters the battlefield, target creature gets +1/+1 until end of turn.
Soulbound - Creatures you control get +1/+1 if there are four or more Druid cards in your graveyard.
Doomsoul2BR
Enchantment - Demon Soul (R)
When ~ enters the battlefield, destroy target creature.
Soulbound - At the beginning of your upkeep, if there are four or more Demon cards in your graveyard, target opponent sacrifices a creature.
Finder of the Lost1BW
Creature - Human Cleric (R)
If ~ is in a graveyard, it has all creature types.
Sacrifice ~: Search your library for a Soul card and put it onto the battlefield. Shuffle your library.
2/2
I added a support card and a cycle of the enchantments.
IIW: New creature types (that make sense in Magic).
Harmonized Existence5GG
Enchantment (R)
At the beginning of your upkeep, you may have your life total become equal to the number of permanents you control. The world is sustained by all its inhabitants.
And holy crap, we have Touch of The Eternal spoiled probably a few days after I posted this
My Card:
Atlantean Diver
Creature - Merfolk Scout UU
Islandwalk ~ can't attack unless there are four or more islands in play.

Actual card:
Harbor Serpent 4UU
Islandwalk Harbor Serpent can't attack unless there are five or more Islands on the battlefield.

I did a search. There is exactly one card with the phrase "can't attack unless there are". My card was created in 2009. Magic 2011 came out in 2010. Although the purpose is quite different. Mine was an alternative to Serra Avenger: instead of counting turns, you count lands. It is inspired by landhome, but intended for aggro and more effective against other blue decks. On the contrary, they used it to completely replace landhome (cf. Sea Serpent) which is almost always seen on big expensive creatures.
Built Deadbridge Goliath a few months ago. All the costs were the same, but it was an elemental instead of an insect. Related, I built the Scavenge keyword ability for my Post Apocalyptic world... guess that it would be a reused mechanic now.
To the Gallows (C)

Also created Loner for GDS2, but for someone else's set. Called it Solitude. One of them was almost the same as Fettergeist, though it was 1UU.
Made an equipment called Armaments of the Archangel, though mine was rare and better because of it.
