It needs some fine-tuning development-wise, but given the existence of Gideon Jura, I don't see how this couldn't work.

Seems awfully bad, considering that basically every blue, red, green, or white spell ever printed basically kills you for one mana. Additionally, the Phage clauses seem kind of redundant--if you're a 20/20, why the heck do you need to destroy creatures when you hit them? Same goes for hitting other players.
Also there are some pretty bad rules things going on here. If you're a 20/20 and I blaze your creature self for ten, does that deal ten to the player, making it a 10/10 with ten damage marked on it? Can creatures attack that creature specifically since its you? Do you even exist anymore as a player or are you just a creature? If I cast Lash Out on your guy and win the clash, do you take six damage or three total? Can I still target you with discard spells or not since you're a creature?
Basically, there are a LOT of reasons this doesn't work, without making at least a dozen rules just to support one card.
Just...no. Just make Serra Avatar with indestructible and auto-game-loss built in. Actually, just don't.

I guess you could play this on turn 5 via High Tide and still have enough mana for the Distortion Strike you'd need to actually do something with this and not die of embarrassment next turn to freaking Unsummon.
I feel like this can be improved by losing the cute "You are a creature" wording.
Maybe there's something between Phage the Untouchable and Form of the Dragon that makes this idea work, but your current execution is more likely to be a game loss than a dramatic 7-drop that shifts the board in your favor, so, back to the drawing board.

It's not a triggered ability. It's a replacement effect and those can't be stifled.

It's obvious to say the card card is not supported by the rules for the simple fact that creatures are permanents and need to be a card or a token (and players are either).
OK so I have a Platinum Angel on the battlefield, now I attack my opponent and they Condemn me. What now?
The leaving the battlefield is replaced with "You lose." Platinum Angel says "Sorry, you can't lose," so it just doesn't happen. All that happens is you gain some life.

Since you're still a player, yes, you can be targeted with discard.

The other way to go around it would to make a card that creates a creature token and gives it all these clauses.
