If a player would draw a card except the first one he or she draws in a draw step, that player discards a card instead. If the player discards a card, he or she draws a card. If the player doesn’t discard a card, he or she puts the top card of his or her library into his or her graveyard.
Whenever a player would draw a card beyond the first each turn, he or she may discard a card. If that player doesn't, he or she puts the top card of his or her library into his or her graveyard.
Since you'll be drawing a card if you discard a card anyway, the conditions are merged together.
This card reminds me of Chains of Mephistopheles which is pretty good - though this isn't as good, since it doesn't force the discard. All in all, a flavorful and balanced card I like a lot.

Actually, Occam's Razor has solved the mystery. Because the card IS chains of Mephistopheles. I just took the Oracle wording for that and made it a card, because I thought the oracle wording was crap.
Also notice that Gerg's revision of the card made the discard optional whereas Chains seems to make it forced...
PlanesJaywalker totally changed the card beyond what it originally was... PlanesJaywalker's "revision" allows this ability to trigger on the draw during the draw step... Also, the part about drawing a card after discarding is omitted... Finally, PlanesJaywalker worded it like a triggered ability rather than a replacement ability which it is...
Also note that I'm commenting on 'Unceasing Torment', not Chains of Mephistopheles (I could say I already knew it was Chains, but it seems to be too late by now ;))
I'm one who goes for the simplest wording possible. If they ask me to redesign Chains, that's how I'll do it.
