Panglacial Wurm is terrible precedence and really a headache, and that's not how madness works (it's a replacement effect coupled with a triggered ability - you don't cast it until the other effect that made you discard finishes up).
That said, if you really want this to be a thing, maybe it should be worded more appropriately?

I'm not sure if that second sentence works, but yours does not (because the other cards you're looking at are still technically the "top two cards of your library" unless you're only scrying 1.

Your trinket text isn't really flavourful, but then again, neither is your card. arbitraryarmor: Flavourful and mechanically interesting. This is great! Certainly a contender. I guess I should probably judge before going to sleep Target. I'm not in the best mood, so my apologies if my judgings are terse.
Lapson2 -No. I really dislike spells that do nothing, and the fact that it also has flashback makes it even worse.
arbitraryarmor - I don't like that this always returns to your hand (barring being countered), and therefore can be used as either a repeatable removal spell or a repeatable heroic enabler (or, you know, for the fact that it is an instant that increases power and grants wither (???)).
willows - This is pretty cute! I definitely smirked when I saw this. I *kinda* wish that it was "up to one target creature" so you can use it to play a land in a pinch w/o creatures, but we're splitting hairs at that point.
Megiddo - This is also pretty cute. I like random stuff (Norin the Wary EDH what what), and the fact that it also recurs itself if you're really lucky is extra fun.
swishh - I don't see how this relates to the challenge at all.
Cardz5000 - This seems clunky. Everything except the last line of rules text is fine, but why can't this just be a normal triggered ability instead of a really awkward replacement effect? The thing already has hexproof, so unless your playgroup has an extreme density of [card=Elesh Norn, Grand Cenobite]Elesh Norn[/c] or Jund Charm running rampant, this thing isn't going to get picked off.
AmShegar - See some combination of Lapson2/arbitraryarmor. While neither of the two above judgments is completely accurate for this entry, and I feel this entry is more useful than the former and fair than the latter, it finds itself in this weird space where when you read it, it feels like there's little gain other than to function as a mini-Bioshift.
Small Adult - I'm not really sure how to gauge this card. I feel like maybe it's not expensive enough, but it's suuuuper swingy. It's either an unexciting pump spell (just +2/+2) or it's a one-creature blowout (+2/+2, double strike, untap, maybe something else - this is a heroic challenge after all). The fact that it works for any creature with a +1/+1 counter on it keeps it versatile, but I think that still just highlights how strong it is. If this were to see print, it would need to be a sorcery.
Winterspring -Yeah sure. Not super thrilled by the recursion everyone is going for, but I guess it makes sense. This one seemed less of an issue at first due to being an Aura and that your opponent can catch you with your pants down, but it has flash too, so
MagicBrains - Replicate doesn't interact with heroic, because you copy a spell, but the copy isn't cast.
avatarz - Well played. I like how you got around the copying a spell issue by copying the card and casting it. Maybe the last word of the last sentence should be "copy" instead of "spell" just to be more clear, though. Also, the +1/+1 boost seems kinda jammed in there.

CrustaceanCrusader - Sooo cute! I like that you still have to pay for the card, so it's not bonkers. I also like that once you cast it, it goes back to the graveyard, so you have to invest another turn to set it up again.
